Community Learning Disability Nurse - Complex Physical Health FTC/Secondment - 12 months Full Time - 37.5 hours per week
This is an exciting opportunity for a learning disability nurse or equivalent to gain experience in a well established community learning disability team for a 12 month period to cover maternity leave.
Main duties of the job
We are looking for a learning disability nurse or equivalent that is passionate about supporting people with complex physical health needs and learning disabilities. You will work as part of an experienced multi-disciplinary team, supporting people whose learning disability compounds the challenge in managing complex physical health issues. The primary focus of the role will be to reduce the risk of admission to hospital and/or premature death, through effective engagement with people with a learning disability and their support networks, such as family members, carers, primary care, social services, acute general hospital care and a range of other important stakeholders.
A learning disability qualification is desirable however we will consider applications from all qualified nurses who have the relevant experience.

- Managing a caseload of people with learning disabilities who have complex physical health needs.
- Engaging with Primary Care services to support the national role out of annual health checks.
- Supporting colleagues in acute hospital services re: effective discharge planning and risk assessment.
- Care planning, crisis planning and risk assessment with appropriate interventions relevant to assessed need.

- Additional specific training in meeting the physical health needs of people with learning disabilities
- Car Driver with access to a vehicle
- Must be able to demonstrate baseline physical health assessments
- Evidence of interoperating physical health assessments and developing health care plans accordingly.
- Sufficient Post Registration Experience related to role
- Community Learning Disability Team Experience
