We are delighted to offer a fantastic opportunity for a HCPC Registered Occupational Therapist to join our team and work as part of our Tamworth & Lichfield Discharge To Assess (D2A) Team, a well-established, diverse and supportive integrated multi-disciplinary team of Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners, Social Care Practitioners, Home First Support Workers and administrators.
This opportunity is currently a static post in our team but we work with other MPFT teams and system partners to support rotations which may in the future offer you a great experience and exposure to other specialities.

Main duties of the job
As part of this role you will focus on providing comprehensive assessments and rehabilitation to support the individuals you see following their discharge home from hospital to maximise their potential, improve their independence and reduce the need for their reliance on long term care and support.
You will be expected to:
Undertake a range of occupational therapy assessments to support maximising patients' independence after a stay in Hospital.
Develop therapy goal plans for individuals which will then be shared with the designated care team to follow.
Review of the goal plans and adapting them to meet the needs of the individual.
Liaise with other professionals in the team around individual patients.
