Bradford District Care NHS Foundation Trust (BDCT) has been proud to host the award-winning Proactive care team since September 2020. Using a lead provider model , BDCT works in partnership with the neighboring acute trust Hospitals, Primary Care Networks and Voluntary sector partners to create a proactive, culturally competent and, aware Multidisciplinary team. Following the success of working in Central Bradford we are now in the fortunate position to roll out a second PACT team in the Keighley locality. We are seeking enthusiastic, motivated colleagues to join us on this next exciting development stage to support a proactive model of working, to assist those most in need and reduce the impacts on primary and secondary care. Applicants will need excellent communication skills, a good understanding of the health care agenda and a vision for service development and growth.

Main duties of the job

As a member of the Proactive Care team working as an Advanced Clinical practitioner in the community setting. Work collaboratively in providing flexible, dynamic and innovative care, responsive to health needs, focusing on those service users who present with complex needs, co-morbidities and long term conditions. To proactively manage a caseload of very high intensity users of services, co-ordinating care programmes to enable the patient to maintain independence, avoid unnecessary hospital admission and facilitate early discharge. To seek and develop innovative and pioneering ways of working, challenging decisions made by health care professionals. To establish close working relationships with staff in primary, secondary, intermediate care, social care and voluntary organisations

Person Specification

Experience

Essential

  • minimum of 5 years post registration experience, minimum of 2 years working at Band 7 or equivalent
  • Experience of working with people with complex health needs in a community setting


Desirable

  • Excellent communication and interpersonal skills
  • Ability to communicate highly complex information where there are barriers to understanding


Qualifications

Essential

  • current professional registration - NMC or HCPC
  • Evidence of continued professional development
  • Completed accredited Advanced Practitioner course which enables ACP registration
  • Independent non-medical Prescriber


Desirable

  • Post graduate study relating to long term conditions, frailty, complex care or equivalent
