Our aim is to provide high-quality, patient-centred care in the comfort of their own homes, reducing hospital admissions & length of stay in hospital & enhancing overall well-being through enhanced care coordination.

Main duties of the job
As Advanced Care Practitioner, you will play a critical role in our new multidisciplinary team, covering one or more of the 6 new frailty virtual wards that we have planned for our locality. The post holder will work closely with consultants, pharmacists, dietitians, and other healthcare professionals to ensure seamless, integrated care on the virtual ward, by providing specialist ACP assessments and interventions for those admitted to the Frailty Virtual Ward.

The post holder will and in accordance with Multi-professional framework for advanced clinical practice in England their professional code of practice and legal frameworks such as non-medical prescribing. Advanced Clinical Practice a level of practice characterised by a high degree of autonomy and complex decision making. This is underpinned by a masters level award (L7) that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence.
Working autonomously as an Advanced Clinical Practitioner within the clinical team and provide a high level of expertise to ensure patient-centred clinical care in the most appropriate clinical setting Exercise advanced clinical expertise, levels of judgement, discretion and decision making whilst undertaking the skills of assessment, examination to provide a diagnosis and formulate timely treatment plans within an agreed scope of practice
Evaluate investigations and revise treatment plans according to patient need and assess impact and outcome.
Assist in the safe referral and discharge of patients with undifferentiated and undiagnosed presentations in any areas of working
Provide expert professional advice to patients, carers and colleagues
Monitor and lead improvements to standards of care through supervision of practice, clinical audit, implementation of evidence based practice, teaching and support of colleagues and the provision of professional leadership.
Lead the implementation of new evidence based practice and contribute to the development of the evidence through research, audit and collaboration with clinical or academic staff.
Contribute to clinical governance by leading quality improvement, audit and research projects as part of the multidisciplinary service-wide performance and quality agenda.

4. SCOPE AND RANGE
The post holder will be an autonomous registered clinician who is confident to work as the only clinician when required and with a consultant/specialty doctor/GP on other occasions.
The Advanced Clinical Practitioner will work within a defined scope of practice and to agreed clinical standards In line with HEE ACP framework (2017)
The post holder will provide a high level of expert clinical care, promoting clinical leadership and set a good standard of professional practice with the organisation
The post holder provides a visible, accessible and authoritative presence within Livewell services and community settings.
The post holder will be working across organisations and communicating with a range of professionals, patients/clients and their families.
Responsibilities will include managing whole episodes of care by supporting medical colleagues, history taking, clinical assessment, diagnosis, treatment including prescribing.
The ACP will be responsible for ordering and acting on investigations, creating management plans, onward referral and evaluation of care
The post holder will lead the development of non-medical led models of care across community services, in collaboration with multi-professional and systems colleagues.
5. MAIN DUTIES/RESPONSIBILITIES
The ACP will be based in the specified Team and works within other related working areas across the specified clinical pathway.
The post holder will:
Proactively support the development of the ACP role within the team primarily focusing on the holistic management of patients within the clinical caseload and service model. Focussing on key areas for example (not exhaustive) admission avoidance to positive discharge, reducing length of stay, and delivering quality patient care. This could be through community management and inreach work.
Actively participate in professional leadership and education across the nursing, medical and therapy teams.
Actively participate in and represent the Organisation in the Regional networks
Actively participating in developing the service.
