Are you looking for a challenging new role? We have an exciting opportunity for a mental health nurse or level 2 social worker to join the new Urgent and Crisis service that was recently launched in April 2024

Do you have a desire to work within a fast-paced crisis network?

The urgent and crisis service supports people who are experiencing a crisis with their mental health and / or need urgent help.

Once a call is made to the urgent and crisis team, the mental health professionals will, where appropriate, arrange a crisis assessment and intervention from the team within 4 - 72 hours using the UK triage rating scale.

Main duties of the job

o To triage urgent/crisis mental health referrals from a variety of sources and to establish an appropriate response.o To offer mental health assessment, formulation and time limited treatment planning for individuals needing an urgent/crisis responseo To provide a resource of highly specialised, time limited interventions to individuals for up to 72 hours following assessment.o To work with other disciplines and teams within Sheffield Health and Social Care (SHSC) to ensure service users benefit from a multi-disciplinary approach that best matches clinical need.Job descriptionWorking together for service users Respect & kindness Everyone countsCommitment to quality Improving liveso To work flexibly to develop team working and to ensure consistent standards of service delivery throughout wider services.o To actively participate in developing team function through tests of change, measuring outcomes, audit and service evaluation.o To work in co-production with service users, providing a high quality and flexible service to meet individual's needs.o To develop strong links and protocols with other services to ensure smooth and timely transitions for patients moving between services

About us

Working x3 shifts a week (12.5 hrs shifts) allowing a better work life balance, in addition to unsocial enhancements for shift work.

Shifts include:-

Twilight - 14:50hrs - 04:00

Night - 19:20 - 08:30

Weekend/ BH days - 07:20-20:30

Job description

Job responsibilities

To undertake the triaging of mental health referrals meeting the threshold for urgent/crisis service and to establish an appropriate response. To undertake mental health Crisis assessments, formulation and time limited treatment planning for individuals needing an urgent/crisis response To conduct psychosocial mental health and risk assessments in accordance with service policy To develop close working links with other mental health teams and agencies to enable swift and appropriate transfer between services. To contribute to and deliver time limited, co-productive care plans in consultation with service users, families/carers, community teams, primary care and other appropriate service providers. To provide comprehensive written reports as required. To participate in clinical supervision both on an individual basis and team/peer supervision. Also to engage in delivering supervision to more junior members of the team. Maintain clinical records according to professional and Trust standards. To attend and contribute to clinical meetings within SHSC. To work flexibly and provide cover in the absence of other clinicians within urgent & crisis service.
