As the Community Palliative and End of Life Care Lead, you will support the Trust's end-of-life care portfolio, coordinating and leading all aspects of our end-of-life care programme in the Richmond community. In this influential role, you will bridge connections with healthcare and community partners, ensuring a seamless, compassionate experience for patients and their families. Your leadership will guide the Trust's strategy, policies, and governance framework to ensure our services remain aligned with national standards and deliver care that is safe, effective, and responsive.

Main duties of the job

o Lead the development, maintenance, and implementation of the Trust's end-of-life care strategy, ensuring alignment with national guidelines and addressing local needs.

o Collaborate with clinical leaders across the Trust and partner organizations to drive a high-quality, coordinated approach to palliative and end-of-life care.

o Develop and monitor key performance indicators (KPIs) for quality assurance and performance evaluation.

o Support the governance framework for end-of-life care by coordinating clinical audits and ensuring the risk register is current.

o Facilitate training and professional development for clinical and support staff, fostering best practices in end-of-life care.

o Act as the Trust's clinical leader for end-of-life care, guiding the development of policies, pathways, and initiatives to enhance patient and carer experiences.

Job description
Job responsibilities

Work with senior clinical leads within the Trust and local health and social care community to ensure delivery and development of the local palliative and end of life programme.

Maintain and develop the Trusts end of life strategy and related policies in line with national guidelines and standards and identified local need.

Ensure best practice is incorporated into end-of-life care plan and work with colleagues to ensure guidance and support for staff is up to date and readily available

Ensure the maintenance of the governance and assurance framework for end of life care

Provide trust wide clinical leadership in end-of-life care and lead the development of the Trusts response to National palliative and end of life care strategies

Support the development of key strategies aligned to end of life care dementia, long term conditions frailty in line with national guidelines and local requirements

Develop and review the palliative and end of life care and associated policies within the Trust and ensure awareness in all clinical services

Ensure that a robust and efficient method of clinical activity recording, in line with National guidelines and recommendations, is in place

Work with partner organisations to develop a forum to develop seamless care for patients

Coordinate the reporting of performance for end-of-life care and ensure reports are timely and clear to support decision making

Person Specification
Education/qualifications
Essential
  • Registered Nurse with current registration
  • Masters level education/qualification
Desirable
  • NMC Teaching qualification
  • Specialist palliative/end of life care qualification
Experience
Essential
  • Knowledge of care and treatment in palliative and end of life care
  • Evidence of incorportating research and evidence into practice
  • Evidence of working in a senior nursing or managerial role
  • Experience of managing organisational change
  • Evidence of achieving key targets within tight timescales
Desirable
  • Experience of working clinically in field of palliative and end of life care
  • Experience of working with a Trust Board and external organisations
Skills/Abilities
Essential
  • Ability to develop and interpret complex clinical information and national guidance and tailor the information to different audiences
  • Highly developed communication skills
  • Ability to lead and motivate others both within and external to the Trust without direct line management responsibility
  • Experience of clinical audit and the ability to interpret results and recommend changes to care delivery/processes
  • Ability to prioritise workload and manage competing demands
Knowledge
Essential
  • Knowledge of care and treatment interventions for end of life care and application within the care delivery setting
  • Knowledge of NHS organisations
Desirable
  • Knowledge of national policy for end of life care
