Main duties of the job

What you will be doing
  • Assessing new patients for nursing care in their own homes, creating and evaluating care plans.
  • Undertaking skilled nursing care for a devolved caseload with support of the District Nurse
  • Support the District Nurse with leading the team on a day to day basis
  • Our core service hours are 8am - 8pm, 7 days per week.

Person Specification

Qualifications

Essential

  • RGN/RN part 1 or 2 on NMC Register
  • Teaching and assessing qualification/Supporting Learners in Practice or current equivalent
  • Educated to diploma/degree level and relevant equivalent training and experience


Desirable

  • Prepared to undertake Community Specialist Practice Degree
  • Nurse Prescriber or prepared to undertake the course


Experience

Essential

  • Minimum three years post registration clinical experience
  • Relevant transferable skills to work in a community setting
  • Experience of working autonomously and managing own patient workload
  • Teaching in a clinical setting
  • Evidence of Leadership Skills


Desirable

  • Long Term Condition qualification/experience
  • Recruitment and Selection experience
  • Experience of managing change and developing teams
  • Minimum of two years working within a Community environment
