  • Manage a caseload of patients and provide Specialist Nurse/ Practitioner support at peripheral clinics as required (Newcastle, Leeds, Sheffield, Liverpool). This will involve remote working or travelling to other hospitals and main base will be in Leeds.
  • Work collaboratively with the multidisciplinary and oncology team to support patients receiving Bevacizumab treatment in their local oncology centers.
  • Responsible for coordination of care. Being the primary patient contact,provide supporting information related to their condition for employment, education and personal independence support. Patient group includes both adults and children.
  • Responsible for communicating highly sensitive and age specific information to patients.
  • Coordinates Patient information for local, regional and national NF2 MDT meetings using EPR-Hive systems and NF2 Database. Link in with NF2 related charities.
  • Liaise with local and peripheral Neurosurgery, Radiosurgery and Oncology teams to co-ordinate specific investigations, clinical reviews, Bevacizumab treatment and data collection.
  • Working with Neurosurgical nurse leads at Salford Royal NHS Trust to ensure adequate preparation of patients for surgery and in-patient support as necessary.
  • To work collaboratively with health professional and social care providers,providingadvice and information on NF2 to medical colleagues and the public.
  • Active involvement in service development, audit and research studies.


Main duties of the job

To be professionally accountable for all work undertaken and to practice at an advanced level of professional autonomy and accountability that is within NF2 guidelines, MFT Trust policies and professional code of conduct

To provide expert specialist advice and support to patients and their families and other healthcare professionals following diagnosis of NF2. Link in with NF2 charities to support and organize patient public events.

To independently receive and make referrals and where appropriate order diagnostic tests in keeping with NF2 guidelines.

Perform a health assessment including obtaining a clinical history, family history, conducting a physical examination and assessment of psychological well being.

To initiate NF2 assessment either in the clinic or in patient' home as appropriate.

To adapt practice according to changing needs of the service and be updated and actively contribute to development new electronic record systems and databases.

Use highly developed communication skills to empower patients to cope with living with a long-term condition.

To be able to initiate the process of genetic counselling with clients and to determine when a couple need referral to a Genetic counsellor

Assessment of patients of all ages with regards to social care, including liaising with social services and the wider social care setting.
