Support the Senior Nurse in delivering and supporting effective care to patients.

Assist with managing the healthcare service across the socially vulnerable groups that we support.

To deputise for nursing colleagues during any periods of absence.

To represent the primary and community care viewpoint at operational and strategic meetings when necessary.

Work closely with the wider multidisciplinary team to ensure safe effective delivery of health care, including mental health and public health services.

To act as an accountable role model and clinical expert for healthcare

Responsibility for the promotion and use of the audit cycle within clinical practice.

Support the development and implementation of Healthcare Policies and procedures.

Responsibility of offering the initial assessment/completion of the All Wales Blue book to Asylum seekers living in ABUHB Locality.

Responsibility of providing cultural awareness training and needs of the socially vulnerable groups to other disciplines for example g.p practices staff, mental health teams and Accident and Emergency staff. Work in conjunction with other disciplines in the assessment, planning, implementation and evaluation of healthcare service provision to ensure a smooth patient pathway through the services.

Main duties of the job

The post holder is responsible for the delivery of nursing practice within the socially vulnerable communities and ensuring that everyone is treated with dignity and needs are met in a timely manner. The focus of the role is to bridge the gap of health inequalities and provide education to the communities and professionals of the social determinants within vulnerable groups.

To develop and maintain close relationships with Primary and Secondary Mental health teams and staff from a range of statutory and non-statutory services, providing advice and support to promote the management of those with mental health needs.

As an autonomous practitioner the post holder is responsible for the care delivered, demonstrating critical thinking, sensitivity and skills in clinical decision-making in the management of patients. You'll ideally have previous experience in a community setting with a knowledge and understanding of culturally sensitive trauma informed approaches.

Qualifications and Experience

Essential

  • Registered Mental Health/Learning Disabilities Nurse with NMC
  • Relevant post registration experience
  • Evidence of achievement in clinical practice within the area of speciality eg primary care, community mental health, older adults, CAMHS.
  • Experience of delivering psychological interventions
  • Knowledge of health inequalities faced by socially vulnerable groups


Desirable

  • Teaching qualification
  • Trauma Training
  • Experience of delivering psychological intervention for example Narrative Exposure Therapy, EMDR or GTEP


Experience and Knowledge

Essential

  • Experience of managing own caseload, time and working independently
  • Knowledge of public health targets


Desirable

  • Evidence of mentorship training
  • Knowledge of wider health economy awareness
