Are you an experienced Registered Mental Health Practitioner looking for a new challenge based in a Community Mental Health Team? Our forward-thinking service has introduced an exciting new post to truly make a difference to the communities we care for...

As the care co-ordinator you will be the first point of contact for patients referred to our service; we care for working age adults from our central base at The Julian Hospital site in Norwich.

In this vital role, you will be responsible for assessing the service user's mental health presentation and risks. You'll be completing initial assessments for service users who are referred into the service and will support triage to ensure they're seen by the appropriate clinicians and services.

We're looking for someone who is passionate, a great listener and is happy to work both autonomously and as part of a wider team. Ideally you'll have experience in community mental health and assessment, although you can expect a thorough induction and training when you start.

It is the ideal time to join our collaborative team as we transform and develop the way we work; we have new clinical leadership roles imbedded within teams which increases the clinical support available, our management team is incredibly supportive and we're passionate about providing training and development opportunities.

Main duties of the job

As an assessor, you will be responsible for completing relevant documentation collaboratively with the service user and any other relevant healthcare professional and identified carers. You'll work together with external agencies and carers in the assessment process, to ensure the service user is referred on to the best team to aid their recovery journey.

We're a friendly, hardworking team who enjoy supporting others; you can expect to work with the full multi-disciplinary team as well as students from a variety of disciplines. You'll be based in Norwich and may sometimes be required to visit service users in their homes to carry out their initial assessment. Although this is a full time role, we can be flexible and can offer some hybrid working and/or flexible working patterns if needed.

To be successful in this role, you'll need to be an excellent communicator, positive and proactive. You should be adaptable and innovative and have the desire to help improve and develop the service we provide - if this sounds like you, we would love to hear from you!

Norfolk and Suffolk NHS Foundation Trust is the main provider of specialist mental health services for the counties of Norfolk and Suffolk, serving a population of 1.6 million people.
