The role of occupational therapists in primary care is continuing to growand we are looking for an experienced occupational therapist to be based in South West Shropshire.
The post holder will be based in one of the practices within South West Shropshire PCN becoming an integral part of the primary care practitioner team, with a focus on working with people whose mental health difficulties are having an impact on their ability to engage in their occupationsof daily living. The post holder will offer a range of interventions including risk assessment for acute distress, developing personalised care plans to support self management, patient activation, social prescribing and sign posting / referral on and use of AHP Health and Work report
