An exciting opportunity has arisen within the Older Adult Mental Health Community services, where we are looking to recruit band 6 CPN to the West Community Hub.
The post holder will care co-ordinate a caseload of patients with complex mental health problems, within Older Adult Services. They should have a good understanding of the CPA framework and our responsibilities within it. They will work along side and within the wider multi-disciplinary team.
They will have experience of working within the Older Adult Services.
Main duties of the job
They must be able to work independently and have the ability to prioritise and effectively manage their workload. The role requires close working with other members of our team, outside teams/agencies, and with our service users and families. The applicant will therefore need to be able to communicate effectively, and have skill with those who may be more difficult to engage.
The post holder will also be expected at times to cover other roles for periods of leave/absence, and will be expected to contribute to Trust/team initiatives as required.
The applicant must have a flexible and 'can do' approach, with the ability to work under pressure and adapt to changes,often at short notice. A positive attitude is essentialforeffective team work and management of the challenges and complexities within our work.
