We are currently recruiting for a Band 6 Care Coordinator to join our friendly Integrated Outreach Team based in Bradford .
You must be either a nurse, social worker or occupational therapist with excellent interpersonal and organisational skills.
As part of the role you will be expected to manage a case load , be committed to team working and deliver skilled interventions aimed at promoting recovery, to people who have enduring, moderate to severe psychotic or affective symptoms with unstable, chaotic lifestyles.Many of our service users also have coexisting substance misuse.
Experience in working with multi-agency staff is essential as are excellent communication skills and a good working knowledge of the Care Programme Approach and Mental Health Act/Mental Capacity Act.
Main duties of the job
The Care coordinator will provide community-based care to clients with complex mental health needs who have been referred into the Integrated Outreach Team.
They will work within a team framework managing and prioritising an individual caseload and will act as a care co-ordinator within the Care Programme Approach.
The post holder will be responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care for a defined caseload of people with complex mental health and/or social care needs.
Service users within the Integrated Outrech team are typically people who need a determined, assertive, and creative approach. Their primary problem is a vulnerability to psychosis many will also experience difficulties related to co-existing problematic substance misuse.
The post holder will provide high standards of skilled mental health care to or service users and their families within a defined geographical area.
The post holder will provide clinical supervision and support to junior members of the team and actively promote the values and policies of the service and the Trust. The care coordinator will be an active member of the multi-disciplinary team and will work in close co-operation with secondary and primary care based professionals, as well as other community agencies such as Housing, Probation, Police, Children Schools & Families and other Voluntary and Third Sector Agencies.
