We currently have a permanent vacancy for a full-time band 6 nurse to work in the Older People's Community Mental Health Team in Heywood, Middleton and Rochdale (formerly known as the MERIT Team).

The team is comprised of community mental health nurses, an occupational therapist, team manager and a team administrator.

The service aims to meet the needs of people experiencing serious mental health problems in the community who are over 65 years of age and people under 65 who are diagnosed with early onset dementia, as well as their families and carers.

We are looking for an enthusiastic, driven nurse, who is committed to working with this client group and being an integral part of our multidisciplinary team.

Main duties of the job

To undertake the role of care co-ordinator in the assessment, treatment, and delivery of care to older people with serious mental health problems, who meet the criteria for access to the service. You will ensure that the highest quality care is carried out in accordance with the policies and procedures of Pennine Care and the Nursing and Midwifery Council.

Job description

Job responsibilities

To undertake holistic mental health and risk assessments using trust approved documentation and procedures.To manage a caseload of clients who will have a variety of mental health conditions, both organic and functional.To provide evidence based therapeutic interventions promoting enablement, choice, and recovery.

To comply with all legislative requirements in relation to service users care.

The post holder will support the duty system that is operated within the team and respond to crisis calls they receive.

Actively participate in multi-disciplinary meetings and contribute to clinical decision making. Engage and supervise student nurses.

To undertake any other reasonable duty which is appropriate to the band, when requested by senior staff.

To be familiar with and comply with all Trust and departmental policies, procedures, protocols, and guidelines.

To be aware of and work towards the Trusts strategic goals.

This role does not involve commissioning services on behalf of the local authority.

Person Specification

Education / Qualifications

Essential

  • RMN or Equivalent level professional qualification
  • Educated to Degree level. Evidence of advanced academia
  • Evidence of further education in Dementia Training
  • NMC registration


Desirable

  • Mentorship


Work Related Circumstances

Essential

  • Full driving licence and access to a car or access to means of transport
  • Willing to carry out all duties and responsibilities of the post in accordance with the Trust's Equal Opportunities and Equality and Diversity policies
  • Appointments to regulated and controlled activities require an enhanced DBS disclosure.
  • Keen interest in working with this client group

  • Evidence of ability to provide specialist mental health assessments, risk assessment and be accountable for decision making
  • IT Skills

  • Ability to provide theraputic interventions for this client group

Knowledge

Essential

  • Knowledge of screening tools and interventions appropriate to older people
  • Knowledge of current best practice in dementia care including management of behaviour that challenges
  • Ability to advise nursing and medical staff in best practice
  • Impact of physical ill health on the mental health of older people
  • Safeguarding procedures and practice
  • Common medications in use with this client group , issues with concordance, interactions, side effects and theraputic effects.
  • An understanding of Mental Health Act and Menatal Capcity Act Legistlation

  • Experience of working in a community mental health team
