The post holder will provide a specialist addictions service and work with service users to comprehensively assess their needs, identify options to meet those needs, and negotiate and review appropriate care within the parameters of ABSDAS eligibility criteria and GP surgeries participating in the GP Shared Care Scheme.

The aim of the service is to support service users with more stable needs who may be prescribed appropriate substitute medication to alleviate withdrawal and cravings, to reduce the use of illicit or non-prescribed drugs until they can achieve a drug free life, and / or to engage in meaningful psychosocial interventions, continued harm reduction education and including the cessation of any criminal and / or risk-taking behaviour.

The post holder works as part of the multi-disciplinary, multi-agency service, supporting and providing a range of treatment and psychosocial interventions for service users and their families / carers / advocates.

The post holder will work closely with GP's, Practice Nurses, Health Visitors and District Nurses, plus liaise with hospital wards and units (A& E, Hepatology etc). On occasion the post holder will liaise with the Complex Services Team in relation to specific needs e.g. Maternity services, Community Mental Health Teams (CMHTs) and Criminal Justice services.

Main duties of the job

To develop agreed Care Plans following the completion of specialist comprehensive assessments, leading to treatment planning, implementation and evaluation of treatment programmes. Deliver harm reduction information and assess physical, psychological and social health and wellbeing, along with other nursing / therapeutic interventions, taking action as appropriate and necessary.

Plan, implement and evaluate treatment programmes.

Negotiate, develop, implement and review Care Plans and identify any potential risks with the service user, their families / carers / advocates and prescribing GP as appropriate.

Deliver harm reduction information and continually assess physical, psychological and social health and wellbeing needs, taking action as necessary.

Provide treatment option information and education to facilitate informed choice.

As part of the assessment and regular monitoring of service users, to participate in the collection of biological samples (e.g. urinalysis). Plus obtain blood samples and send for analysis ensuring relevant documentation is completed and appropriate protocols are followed, as necessary.

Support and motivate service users to fully engage with treatment programmes and maintain compliance with medication regime.

Support local GP's within the Shared Care Model, liaise with other support agencies, statutory and voluntary, and facilitate transfers where appropriate.
