We are looking for a dedicated, proactive, and highly motivated District Nursing Team Manager to lead our Acton District Nursing/Community Nursing Team , supporting patients within the Acton quadrant of the Ealing Community Partners.
This is a unique opportunity for an experienced clinical leader who is passionate about enhancing community health through the delivery of high-quality, integrated nursing care that improves patients' quality of life.
As part of our ambitious vision for integrated health and social care across Ealing, you'll work closely with a multidisciplinary team--including GPs, District Nurses, Social Workers, Rapid response team, Palliative care team, etc. --to provide holistic, patient-centred care within the Acton community.

Main duties of the job
In this role, you will be responsible for managing and supporting a team of skilled nurses, with a focus on preventing unnecessary hospital admissions and promoting patient independence.
