The post holder will be responsible for providing specialist rehabilitation nursing for patients with neurological conditions attending the outpatient clinics at the West Midlands Rehabilitation Centre (WMRC) and in community settings.

They will work closely with service users, clinical teams and partner organisations to ensure a co-ordinated approach to the management of people with long term neurological conditions.

The Specialist Nurse in Neuro Rehabilitation will have input into all clinics as required, but defined roles and responsibilities within the Multiple Sclerosis, Non Traumatic Spinal and Spasticity/ITB Specialist Clinics.

Main duties of the job

Work as part of the interdisciplinary team to provide specialist assessment and goal planning for patients with complex/multiple pathologies who are referred to the specialist clinics, especially Multiple Sclerosis (MS) and Spasticity/ITB Management rehabilitation clinics.

Provide regular review of patients' conditions and goal setting for patients within the specialist clinics, individually and as part of the team, recognising where appropriate the deteriorating patient conditions and implication of care change required for care management, initiating referral to other agencies e.g., palliative care.

Act as a point of contact for patients who are experiencing clinical problems relating to nursing issues e.g., bowel, bladder, tissue viability, pressure area care and sexual dysfunction.

Provide domiciliary home visits as needed.

Provide cover for the Outpatient Nursing Team as required.

Ensure that patient who are undergoing conscious sedation have appropriate nursing support during and following this procedure and are able to recognise the 'deteriorating patient' and act accordingly.

Deputise for the MS Clinic Lead & Nursing Lead as required.

Participate in triage and decision-making regarding allocation of referred patients to specialist clinics.

To support Medicines Management within West Midlands Rehabilitation Centre as required.

The WMRC Specialist Clinics Team consists of nurses, therapists and an assistant practitioner. They work closely with members of the MDT, including Consultants, Rehabilitation Engineers, Orthotists and Prosthetists.

Person Specification

Qualifications & Training

Essential

  • BSc (Hons) Nursing
  • Current Registration with NMC, Adult Masters Level qualification in Neurological Conditions or ability to demonstrate equivalent knowledge and skills
  • Master's level qualification or equivalent
  • Practice Assessor/ENB 998/SLAiP or equivalent


Desirable

  • Evidence of post graduate training in Multiple Sclerosis such as the MS Trust Specialist Nurse Programme


Experience

Essential

  • Experience of managing patients with Multiple Sclerosis
  • Evidence of professionally autonomous practice
  • Leadership ability
  • Experience of Clinical audit
  • Experience of managing patients with Neurological conditions
  • Experience of managing patients with Spasticity
  • Experience of working in Outpatient setting
  • Experience of Domiciliary visiting


Desirable

  • Experience of managing patients with Intrathecal Baclofen


Skills & Knowledge

Essential

  • Knowledge of specialist neurological rehabilitation, assessment, intervention and highly specialist services especially management of patients with Multiple Sclerosis
  • Knowledge of professional codes of conduct and regulatory requirements
  • Knowledge and experience of clinical audit and research
  • Evidence of good communication and organisational skills
  • Verbal and written communication and negotiations skills appropriate to a range of audiences and situations
  • Ability to develop self and staff
  • Ability to demonstrate knowledge of clinical governance, standard setting and the audit process (understanding of principles of clinical governance)
  • Ability to demonstrate sound knowledge and application of up-to-date clinical trends and practices including evidence-based ones
  • Ability to communicate effectively with patients /carers /colleagues /senior management and other agencies. Interpreters may be used on occasions
  • Knowledge of up-to-date practice, evidence and research in the management of Multiple Sclerosis
  • Knowledge of spasticity management, including spasticity triggers
  • Knowledge of Medicines Management
