Newcastle upon Tyne Hospitals NHS Foundation Trust are seeking to recruit a highly motivated and enthusiastic Band 6 Nurse Practitioner to come and work alongside colleagues within our busy intermediate care unit. Intermediate care supports people to avoid going into hospital or residential care unnecessarily, facilitating people to be as independent as possible after a stay in hospital or a period of not managing within their own home.
Our unit provides high quality care to patients requiring rehabilitation to optimise their health before returning to their usual place of residence. We work as a multidisciplinary team with patients to achieve patient goals and best patient outcomes.

Main duties of the job
The post holder will work as a Nurse Practitioner within the established multidisciplinary team. The Nurse Practitioner will work within a team of nurses to complete the history taking of patients and physical assessment using clinical skills which are a significant part of the job role. If not already possessed a willingness to undertake this study would be essential. Again, prescribing study towards V300 would also be supported for non-medical prescribing.
The team provides medical and therapy services to patients in Eden Court including comprehensive assessment in daily board round and goal planning towards discharge planning. Patients can expect to stay a maximum of six weeks in the rehab setting.

- Will provide a prompt response to calls from colleagues, prioritisingimportance and clinical need.

- Provide immediate care management of patient including communication of plan to patients and significant others.
- Review and refer to appropriate person, e.g. AHP, resident medical staff, specialist registrar or critical care outreach team.
- Liaises with Patient Services Coordinator.
- Communicate within multidisciplinary team, ward/departmental staff and/or out of hours team.
- Contributes to specialist education and training to ward departmental staff, nursing and medical students.

Qualifications & Education
Essential
- Registered Nurse (appropriate to branch)
- Current NMC registration
- Diploma plus specialist qualifications / experience within specialist area prepared to take degree level study
- Working towards / prepared to work towards:
- Nurse Practitioner Qualification or equivalent
- Clinical Skills Course
- Non-Medical prescribing if relevant to role
- Meets Nursing and Midwifery Council standards for student supervision and assessment (formally Mentorship) if relevant to role
Desirable
- Degree in health related subject
- Has undertaken and completed Non-medical independent nurse prescribing course
Knowledge & Experience
Essential
- Extended clinical practice underpinned by training and assessment e.g. venepuncture, cannulation skills and intravenous drug administration
