Norfolk Community Health and Care NHS Trust in West Norfolk have an opportunity for an experienced Band 5 Nurse to join our community team. West IV Team based at St James Clinic, King's Lynn. We are looking for a dynamic and motivated nurse to join our friendly professional , busy team with a passion for community nursing to provide Intravenous Therapy visiting patients in their own home / care homes and community hospital to support early discharge from the Acute Trusts and also assisting with Admission Avoidance.
Experience of IV administration, phlebotomy, PICC/central lines and cannulation is essential as well as being able to work alone.
You will be working within the Intermediate Care Services so may also be required to work alongside the Virtual Ward Community Team based also at St James Clinic, as well as support the community nursing teams when required.

- Supervision will be provided by a Senior Registered Nurse.
- Active role in Community Intravenous Service for patients
- Assess, prescribe and deliver care packages for patients
- Work with integrated team to prevent unnecessary admission to hospital
- Able to triage, review timely responses to referrals on a needs-based basis
