The post holder will provide expertise, leadership and support to health visitors who are working with a child with a disability and who may have special educational needs, and their families for 3 days (0.6 WTE). They will be a member of a Health Visiting (HV) team undertaking a corporate caseload responsibility for any remaining contracted hours up to 2 days (0.4WTE). This may be modified to meet service need.

Main duties of the job

Lead, facilitate and embed a positive, pro-active approach to supporting children with disabilities.

Maintain an up-to-date, citywide overview of all children with disabilities

Co-ordinate and facilitate effective communication between all professionals and partner agencies involved in the care planning of the child and family.

Be an advocate for children with disabilities and their families at strategic, inter-agency working groups

Co-ordinate, update and disseminate information on resources, services, support networks and contacts for children with disabilities to enable health visitors to facilitate timely access to support.

Co-ordinate and provide learning opportunities, expertise and guidance for health visiting teams in order to optimise and safeguard health outcomes for children with disabilities.

Implement, evaluate and audit agreed processes for information sharing and partnership working between health visitors and other agencies involved in managing the child with disabilities.

Co-ordinate and facilitate the seamless handover of care in partnership with key stakeholders when the child starts school.

Person Specification

Education / Qualifications

Essential

  • Registered Health Visitor or Registered Specialist Community Public Health Nurse (SCPHN) degree.
  • Knowledge of specialist pubic health field acquired through relevant degree plus additional training or experience to masters degree or equivalent.


Experience

Essential

  • Experience of working with complex cases with children with additional needs
  • Extensive safeguarding experience and multi displinary case planning
  • Knowledge of early education and SEND agenda
  • Experience of change management and projects
  • Experience of training staff and devising packages of training


Desirable

  • Evidence of management/leadership development
  • Evidence of further development in relation to working with children with disabilities and their families
