This is a Band 6 community mental health nurse role working within Older adult services in Bury. This role involves responsibilty for providing interventions to identified clients in a community setting, involving severe and enduring mental health issues and particularly challenging and complex cases.
Main duties of the job
To work in the community and liaise with in-patient areas assessing andprioritising clients appropriately.To manage a caseload of clients with complex and challenging mental health needs.To assess clients in the most appropriate location often in their own homeworking in accordance with the Lone working Policy.To plan and implement appropriate client centred interventions to meet the needs of the individual. Contribute to making decisions regarding client care in consultation with themulti-disciplinary team.Undertake the assessment, planning, implementation and evaluation of of care.Maintain standards with regards to the quality of interventions provided. Work within the framework of the All Agency Safeguarding Vulnerable AdultsPolicy and be willing to undertake relevant training.To carry out a thorough holistic assessment of the service users' current mental health needs, including risk assessment and psychological needs.To demonstrate the ability to make decisions in relation to the clinicalmanagement of individual patients and make critical clinical judgements inrelation to mental health and care provision within the community settingTo be able to assess and manage clients presenting with co-morbidities ofphysical and mental health needs including the impact of medication.Maintain confidentiality and a high professional standard at all times

You will be joining a full team of highly experienced practitioners which includes Nurses, Older Peoples Clinical psychologist , Support worker, Psychiatrists and Mental Health Well being Practitioner.The role is support the older peoples community mental health nursing team in managing the care and treatment of older people in the community. This will involve working with patients diagnosed with both organic and functional illness. The post-holder will also support the duty system that is operated within the team in response to crisis calls that they receive.
