Are you looking for a new, dynamic challenge developing and improving the transitional care of young people with congenital and acquired heart disease? An exciting opportunity has arisen for an experienced, motivated and enthusiastic nurse to join the cardiac adolescent and transition service.

We are looking for an experienced cardiac nurse with vision and enthusiasm for delivering holistic young person centred care. The role includes service development of transitional care for young people with congenital and acquired heart disease, leading a small, motivated team, whilst managing a caseload of young people with congenital heart disease (CHD) and acquired heart disease. You must have excellent leadership and management skills, experience in service development and improvement and aspire to develop innovative teaching strategies, whilst also being able to quickly develop a positive rapport with young people. The role will involve developing strategies to empower young people to manage their on-going health needs as they prepare for adulthood, whilst implementing pathways to reduce patients becoming lost-to-follow-up.

We will support your on-going development and educational pathway to achieve these goals. This is a full-time position.

Main duties of the job

o Work with cardiac services to develop a network of care including inpatient, outpatient and community support whilst demonstrating a nurse lead in this area of practiceo Work with young people with congenital heart disease (CHD) and acquired heart disease aged 12 years upwards within the Trust, across the cardiac networks, and into the communityo Demonstrate appropriate expert clinical practice and knowledge of this patient group, leadership skills, education, and an ability to use research/audit to develop practice and improve current knowledge baseo Develop clinical judgment and decision making in relation to the management of patients.o Engage, empower, and prepare young people to develop their independence and knowledge of their condition and lifestyle in readiness for adult serviceso Improve the quality of care and the patient journey of young people with CHD and acquired heart disease through GOSH services and across cardiac networks

Essential

  • On the relevant part of the NMC register, RNA / RNC, or equivalent
  • Evidence of ongoing, dynamic continuing professional development within the specialty, linked to demonstrable clinical competencies
  • Possession of a relevant Degree or in the process of obtaining.
  • Leadership or management training
  • Basic life support
  • Specialist congenital heart disease modules/courses undertaken or willingness to undertake
  • Completion of adolescent e-learning for health modules, particularly transition module

Experience/Knowledge

Essential

  • Minimum 2 years at band 6, or equivalent, including within relevant clinical environment
  • Experience of both clinical and formal teaching
  • Experience of working with young people in transition / coaching young people/life skills development
  • Thorough knowledge relevant to the specialty/ department / field of practice
  • Knowledge of adolescent development and lifestyle challenges
  • Knowledge of transition theories and key documents, information resources and national standards e.g. DH (2011) You're Welcome DH (2008) Transition: moving on well DH (2006) Transition: Getting it right for young people CQC (2015) From the Pond Into the Sea NHSE (2016) CHD Standards and Specifications, ESC (2020) ACHD Guidelines, NICE (2016) Transition Guidelines etc.
  • Understanding of professional and current issues around congenital heart disease e.g. Congenital Heart Disease Review
  • Understanding of resource management, health and safety, clinical risk and quality issues
  • Understanding of child protection procedures
  • Awareness of equality and diversity

  • Experience in the use of information technology to promote health and communication


Desirable

  • Experience of leadership abilities e.g. taking charge of a ward area or change management project
  • Experience of leading and managing a team of nurses
