An exciting opportunity has arisen to join the Practice Development Nurse in Area Central Team based in Bron Afon, Llanfairfechan on the North Wales Coast.
The successful candidate will take a lead role on a number of specific work streams relating to Mental Health and Dementia care as well as general nursing care provided in care homes and will work collaboratively across the Central area of Conwy and Denbighshire in order to implement work programmes that are aligned with national and health board wide objectives.
The key principles of the practice development post will be to focus on the improvement of patient care and safety through adoption of a range of approaches. This will involve supporting staff within the care home nursing setting. This may involve incorporating other multi professional areas as defined by the Health Board.
The successful candidate will also be responsible for providing professional and clinical leadership and support within the EMI and general care homes, covering clinical advice, care planning, aspects of raising awareness and teaching on Dementia and mental health issues and physical health issues to all Healthcare Professionals working in all care homes across the two counties.
Main duties of the job
Develop and maintain high effective communication networks with key stakeholders to support and ensure development of professional behaviours and a range of knowledge and skills for all staff.
Develop, facilitate, deliver and evaluate along with other practice development leads and HEI providers, education and training for nurses and health care assistants linked to area needs and relevant standards.
Promote patient safety and reduce clinical risk through the development of educational programs, assessment of clinical skills and clinical competencies.
Support the delivery of a high quality, safe and compassionate health care service, in all aspects of their work and consistently demonstrate BCUHB's purpose and values.
The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.
