The UCR Team, consists of Geriatricians, Advanced Clinical Practitioners, Paramedic, Nurses, OTs, Physios, Associate Practitioner and Therapy Assistants.

We have received significant additional investment to increase the capacity of the Urgent Community Response (UCR) service and are now able to recruit a band 6 Physiotherapist or Occupational Therapist to the existing team.

UCR Therapy focuses on crisis management and admission avoidance to maintain patient safety at home. Essential functional assessments are carried out with prescription of urgent complex equipment, ongoing therapy follow-up visits to ensure patient safety, and short-term support for patients in acute medical crisis. UCR Therapy also responds to non-injury fallers to assist them off the floor using equipment such as the Raizer chair, or Mangar Elk, avoiding 999 calls.

There is a strong teamwork ethos with multi professional visits being carried out to maximise clinical efficiency ensuring patients get the right treatment at the right time at home.

The service is constantly evolving to meet the needs of the patients in our community, and the team is encouraged and supported to be innovate and creative with their service delivery to achieve the best outcome for their patients.

Main duties of the job

1. To undertake clinical assessment, diagnosis and treatment of patients on the caseload. These patients may have acute, complex and/or chronic presentations and may have varied diagnoses, prognoses and rehab potential

2. Urgent, varied caseload with rapid response function to support patients, often picking up unplanned work throughout the day

3. To assess and mitigate immediate risk to patients, either as a result of their clinical condition and/or personal circumstances

4. To ensure immediate patient safety and to work with the patient following this to ensure longer term safety and wellbeing, building on strengths and goals

5. Case management including holistic patient assessments considering all areas of patient need, drawing on extended scope of practice following training and competency sign-off

Job description

Job responsibilities

1. To provide clinical assessment and treatment to patients

2. To develop and ensure the safe delivery of care plans to support patients to achieve their goals

3. To review patients in line with their care plans, goals and changing needs

4. To assess for equipment that will support a patient to be as independent as possible

5. To assess for and implement treatment and rehab plans that will support a patient to be as independent as possible

6. Safe delegation of rehab and treatment of patients to non-registered staff

7. To ensure safe and effective caseload management and appropriate throughput of patients, enabling focused service delivery

8. To manage a varied caseload, seeking support from senior staff where necessary to ensure the provision of an up-to-date, clinically effective service based on evidence of good practice

9. Escalation of patient deterioration

10. To support patient discharges from inpatient settings, and to provide clinical services for patients in crisis for admission avoidance

11. To undertake in-reach to inpatient settings to support discharge into the community

12. To support the clinical development and delivery of Physiotherapy within the community

13. To lead on projects and developments within the locality

14. To contribute to interdisciplinary working by teaching and education of professional skills & techniques as defined by an agreed competency framework or training scheme: personally and for other professionals, students, patients and carers. Where appropriate to take responsibility for the application of those new skills to rehabilitation programmes. e.g. - equipment provision, exercise prescription

15. To provide line management and supervision to junior staff and practice placement students as appropriate

16. To maintain excellent clinical practice through the CPD process, and taking part in relevant research, audit and development activity as directed

17. To work closely and jointly with Occupational Therapy and Nursing colleagues to ensure a holistic and effective care plan is provided to the patient

18. To oversee clinical tasks delegated to junior staff and ensure staff competency

Qualifications

Essential

  • Degree or equivalent qualification
  • HCPC registered


Experience

Essential

  • Significant relevant post registration experience
  • Experience in discharge planning and admission avoidance


Knowledge

Essential

  • Up to date knowledge about best clinical practice in community and rehabilitation
  • Knowledge of key drivers nationally re: integrated care and intermediate care
  • Ability to assess, plan, and evaluate the rehabilitation needs and progress to effective outcomes
