We are currently looking to recruit a band 5 Mental Health Nurse into the Older Peoples Service Community Mental Health Team in the East of the city (based in LS7). We are looking fora committed and motivated practitioner who wishes to consolidate their practice in community services and develop their skills, experience and knowledge.
You must be able to demonstrate an in depth understanding of the needs of Service Users with severe, enduring and complex mental health needs including mood disorders, psychosis and dementia. Working with this special group of service users can bring its own challenges and complexities so a passion and desire to work within this part of the service is essential.
The successful applicant will be expected to demonstrate the capacity to work independently but under the supervision of senior team members. You will receive clinical supervision and mentorship with a robust personal development plan to support and develop your practice.
Main duties of the job
The successful candidate will be responsible for the comprehensive assessment, planning, implementation, and evaluation of individual care needs for this client group. The candidate will have a good knowledge and understanding of how to assess and escalate risk within these settings. There is a requirement to keep up to date records using electronic care systems.
As a band 5 practitioner you will provide professional input into the Multi-Disciplinary team which comprises of professionals from multidisciplinary backgrounds, working collaboratively to provide specialist focused mental health interventions. As an autonomous practitioner you will be required to provide care co-ordination within the framework of the CPA process. Service User involvement is at the centre of our approach, and you will embrace this in your practice. You will be able to demonstrate a range of therapeutic interventions and be committed to the concept of recovery whilst working alongside the wider community provision in Leeds.
You will need to be able to work independently out in the community. It is essential that you have a full UK driving licence and use of a car. The successful applicant will be offered a comprehensive continuing professional development package which will include access to appraisal, clinical and managerial supervision.

Within the Community setting you will be expected to implement all relevant forms of care without direct supervision and may be required to demonstrate procedures to and supervise qualified and unqualified staff. You will act as a mentor to student nurses, trainee nursing associates and support newly qualified staff on preceptorship. You will be expected to regularly act as the duty person to triage referrals to the team and be the point of contact in the absence of colleagues or when further support is indicated.
An understanding of the needs around dementia would be desirable but not essential as the successful candidate will receive training and supervision from a senior practitioner. Post registration experience is not essential and newly qualified or soon to be qualified applicants will be considered.
Service operating hours are Monday to Friday, 09.00-17.00. It would be recommended to contact the services for further information and discussion about the posts. Part time hours will be considered.
