An exciting opportunity has arisen to join an innovative and dynamic team within the Solihull Community Respiratory Team at UHB. Working out of Solihull Hospital the post holder will support all elements of the service which includes hospital at home, admission avoidance (Virtual Ward) Oxygen service (HOSAR) Pulmonary Rehabilitation (PR) Specialist Respiratory Nurse led clinics. This is a community team and the role involves the postholder to be a car driver/user in order to deliver the service across the Solihull geographical area.

We are looking for an experienced and motivated Band 6 Clinical Nurse Specialist who is passionate about service development and providing high standard of patient centred care. You will be working closely within a multidisciplinary team of staff including Respiratory Consultants, hospital based Respiratory Nurse Specialists, Physiotherapists, Therapy Support Workers (TSW) and Occupational Therapists, all of whom are committed to maintaining independence, promoting self-care and assisting patients in their usual place of residence. The successful candidate will provide respiratory intervention, as well as supporting the multidisciplinary team in assessment and development of a holistic management plan.

Main duties of the job

The post holder will perform specialist assessment of respiratory patients with diverse presentations and complex physical and psychological conditions, developing and delivering an individualised treatment plan based on this assessment. This will include patients with COPD, Interstitial Lung Disease, Bronchiectasis, Asthma and palliative patients requiring oxygen. These assessments will be in both the acute and chronic state.

Qualifications

Essential

  • *Diploma / degree in adult nursing
  • *Current registration with the NMC
  • *Evidence of CPD including recent post graduate training relevant to clinical speciality.


Desirable

  • *Evidence of leadership training.
  • *Health Care Assessment Course
  • *Post registration Respiratory course (COPD or Asthma diploma)


Experience

Essential

  • *Demonstrates comprehensive clinical experience enabling the post holder to manage a caseload of patients with complex needs in this speciality.
  • *Significant experience of manage a caseload of patients with complex needs in this speciality. Minimum 12 months post-registration experience, providing a broad range of experience, must include respiratory
  • *Knowledge of COPD NICE Guidelines, COPD GOLD Guidelines Standards of Care, BTS Pulmonary Rehabilitation Guidelines, BTS Bronchiectasis Guidelines
  • *Experience of supporting and supervision of junior staff, students and assistants.
  • *A specialist knowledge across a range of therapy practices, routine and non-routine, requiring expertise within the field of therapy.
  • *Good understanding of working in the community.
  • *Understanding of current legislation and application to patient care


Desirable

  • *Team/service development
  • *Knowledge of BTS Oxygen Guidelines
  • *Knowledge of the Home Oxygen Assessment and Review Service (HOS-AR) recommendations
  • *Evidence of audit / R& D activity
