Main duties of the job

We're looking for a dynamic individual to lead our nursing team and ensure exceptional standards of care while championing health promotion and shared decision-making with patients and their families.

As a key member of our multidisciplinary team, you will not only manage your caseload but also provide inspiring clinical leadership to your colleagues. Your expertise will shine as you collaborate with specialists, particularly in community IV therapy, making a real difference for patients with both acute and long-term conditions.

In this role, you'll monitor progress, assess treatment responses, and implement strategies to reduce unnecessary hospital admissions--all while promoting preventative nursing practices that empower patients to reach their health potential.

Person Specification

Essential & Desirable

Essential

  • Registered Nurse/Adult Branch with current NMC registration
  • Evidence of post qualifying experience
  • Experience of Case / Care management within a Nursing Team.
  • Able to perform concise comprehensive patient holistic assessments,prescribe appropriate nursing care and treatment, implement and evaluate outcomes.
  • Experience of using IT Clinical Management Systems.
  • Ability to travel to different geographical locations across the Trust footprint in line with service needs


Desirable

  • V300 Non-Medical Prescribing qualification or willingness to work towards
  • Ability to manage staff and difficult situations. To be responsible for facilitating student placements.
  • Full UK driving license with willingness to use own car for work purposes
