As the Clinical Operational Lead, you'll drive the delivery of high-quality, efficient, and patient-centred care through the One Community Virtual Ward and operationally oversee the Urgent Crisis Response (UCR) Service. Collaborate with Primary Care, UEC Providers, NimbusCare, and York & Scarborough NHS Foundation Trust to provide cutting-edge solutions for frailty conditions.

Partner with the B8a Therapy Lead to ensure seamless clinical and operational oversight across services.

Main duties of the job

  • Lead & Innovate: Drive the development of care pathways across the Virtual Ward and UCR Service, ensuring seamless coordination of interventions.
  • Urgent Response Optimisation: Spearhead efforts to meet the 2-hour urgent response target, utilising virtual ward capacity and technology-enabled care for enhanced service delivery.
  • Clinical Expertise: Provide expert guidance within the Virtual Ward, offering specialist support to patients, families, and healthcare professionals, while empowering staff to deliver best practices.
  • Collaborate & Expand: Strengthen partnerships across secondary, community, and primary care. Represent the services in regional networks, contributing to shared learning and MDT discussions.
  • Innovate & Expand Services: Lead the growth of Step Up/Step Down services and the integration of tech-enabled care solutions, using QI methodology to drive pathway development.
  • Operational & Strategic Support: Assist the General Manager in shaping strategic plans, ensuring a coordinated approach to pathway services and cohesive health and social care partnerships.
  • Performance Leadership: Ensure optimal clinical and operational performance, supporting the General Manager in achieving and improving KPIs.

Qualifications and Knowledge

Essential

  • Be a member of a recognised professional body and hold a current professional registration.
  • Detailed understanding of relevant policy, legislation, drivers and their application to clinical/service/organisation.
  • Evidence of management, leadership and organisational development issues that have had a positive impact on service delivery.
  • Experience of successful policy implementation and change.


Desirable

  • Advanced management/leadership training/qualification or equivalent experience
  • Member of organisational group/network, locally or nationally.
  • Advanced/expert understanding of management, leadership and organisational development.
  • Educated to Masters Level or be able to demonstrate equivalent knowledge or experiential learning/experience.


Experience

Essential

  • Evidence of sound post-registration professional practice.
  • Evidence of continuing professional development relevant to the professional lead area to advanced level of post-graduate diploma or equivalent knowledge/experiential learning.
  • Proven leadership/management experience at an advanced level which has had a positive impact/crated change within service delivery/practice.
  • Be able to effectively chair meetings.
  • Evidence of managing a budget within set resources.
  • Ability to work across organisational boundaries, developing and maintaining multi-professional and multi-agency partnerships.
  • Full understanding/application of relevant clinical practice/standards/audit within identified clinical area.


Desirable

  • Proven experience of developing others through education, mentorship, coaching, teaching, assessing, presentations, publishing.
  • Evidence of policy implementation and development
  • Advanced management/leadership training/qualification or equivalent experience
