Perform a Specialist Occupational Therapist role to provide a comprehensive Occupational Therapy Service to the patients referred.
This will be a split post to cover 12 hours in Acute Paediatrics & 25.5 hours in Acute Adult Services.
Be part of an interdependent multidisciplinary team, ensuring that Occupational Therapy input is an integral part of the patient's overall care and discharge.

Demonstrate an integrated approach to patient care, working collaboratively with external agencies, such as social care and education.

Responsible for managing children and young people with conditions and providing assessments and equipment for them.

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Main duties of the job

Patients seen present with a wide range of complex and highly complex acute and long-term needs and disabilities.

This role will include the provision of Occupational Therapy Services to Paediatric Patients

This will include working alongside the Physiotherapist within the paediatric department to provide an Occupational Therapy Service to patients on the ward. Provide pre op & post op support to surgical patients & patients with complex diagnoses referred on from other hospitals This will often involve the provision of specialist wheelchair's & liaising closely with other external children's Therapists / services.

May be required to work anywhere in the Trust.

To be an active member of the 7-days working rota, where required.

Job responsibilities

This will be a split post to cover 12 hours in Acute Paediatrics & 25.5 in Acute Adult Services across the medical wards.

Clinical

1. Ensure the patients, relatives and those the therapist meets are aware of the role of the Occupational Therapist and components of the Occupational Therapy treatment programme.

2. Obtain informed consent from the patient and parent, and to work within the current legal framework with patients who lack capacity to consent, prior to commencing any treatment.

3. Responsible to assess for and provide aids, adaptive equipment and wheelchairs to increase / enable or compensate for independent function, teaching compensatory techniques as required to the patient and those involved with their care.

4. Responsible for the assessment, intervention, discharge and ongoing referral of patient referred. The assessments include, but not limited to mobility assessment, transfer assessment, hand function assessment, personal care assessment and assessment of the home situation. select from a wide range of standardised and non-standardised assessments to assess and aid diagnosis of cognitive, perceptual, behavioural, physical and social functioning of the patients referred.

5. Responsible to plan, deliver and review treatment programmes in close liaison with all members of the multi-disciplinary team, carers and family in accordance with any consent given. Whilst actively encourage children participate in both planning and provision of care, when possible.

6. Provide specialist advice to the ward / Occupational Therapy department on complex and highly complex cases

7. To provide Occupational Therapy treatment interventions in a variety of environments including home, school, and other premises in the community.

8. Make reliable interpretations of the assessment results, to inform other professionals in the diagnosis and management of the patients. Also to guide the Occupational Therapy treatment plan, and for the recommendation of the discharge destinations/placements.

9. Responsible to establish robust communication networks with patients, relatives, carers, statutory and voluntary bodies to establish a comprehensive discharge package to ensure continuation of care in the community. This includes providing written communication to support the need for services and equipment, and effective liaison with other hospitals, rehabilitation centres etc. as necessary, when patients are transferred.

10. Plan, deliver and review treatment programmes in close liaison with all members of the multi-disciplinary team, patients, carers and family in accordance with any consent given. Addressing areas such as disease management, sensory, motor, splinting, psychosocial needs and functional abilities.

11. Carry out environmental home assessments, pre-discharge home assessments, discharge home assessments and follow-up visits, as required. Assess the patient carrying out functional skills to maximise independent living, arranging equipment provision, adaptations and support services as required. Assess for disability access and safety, also ensuring access to community resources and assessing carer needs.

12. Assess for and provide aids and adaptive equipment to increase / enable independent function, teaching compensatory techniques as required. These include but are not limited to feeding, dressing, leisure activities and employment. Responsible for providing information and instruction for the safe use of the items provided.

13. To assess for, trial and prescribe, specialist, non-stock items of equipment from specialist suppliers

14. Raise requisitions for minor adaptation on behalf of Social Services and OT equipment on behalf of the OT Department, Derriford Hospital.

15. Assess for wheelchairs, specialist seating and positioning needs treating as appropriate, referring on to wheelchair / seating clinic if required.

16. Complex discharge planning, which involves identifying of patients and carers needs, and the co-ordination of services in the community, to address the diverse and complex presentation and needs of the paediatric patients.

17. Responsible to plan, deliver and review treatment programmes in close liaison with all members of the multi-disciplinary team, patients, carers and family in accordance with any consent given.

18. Provide specialised Early Phase Rehabilitation for patients with any medical condition. This can involve personal activities of daily living, kitchen and eating practice, desensitisation programmes for patients with hypersensitivity, remedial activities to promote overall functioning cognitively, perceptually or physically. Teaching Anxiety management strategies and relaxation techniques.

19. Assess, fabricate and monitor thermoplastic splints for patients as appropriate.

20. Review, record and report regularly on patients treatment and progress to the multi-disciplinary team in accordance with the policies and procedures laid down in individual clinical areas, whilst always ensuring confidentiality.

21. Complex discharge planning, which involves identifying patients and carers needs, and the co-ordination of services in the community, to address the diverse and complex presentation and needs of patients, e.g. arranging the provision of and training the carers for the hoist for discharge.

22. Actively participate, liaise and ensure good communication with the MDT, carers, social services, statutory and voluntary bodies to establish a comprehensive discharge package to ensure continuation of care in the community. This includes providing written communication to support the need for services and equipment.

23. Liaise effectively with other hospitals, rehabilitation centres etc. as necessary, when patients are transferred.

24. Provide Patient/Carer Education/Advice. Ensuring patients understand the detail or complexity of the impact of disease and physical and emotional deficits on patients safety and independence in the hospital, home, leisure and workplace.

25. Use a range of verbal and non-verbal communication tools to communicate effectively with patients. This will include patients who may have difficulties in understanding or communicating, e.g. patient may be depressed, deaf, or who may be unable to accept the diagnosis/prognosis

26. Utilizing interpersonal skills, demonstrating empathy and involving patients in their treatment, in order to achieve the treatment goal.

27. Provide appropriate advice and support to patients following terminal diagnosis in liaison with the MDT and specialist services including the Oncology Specialist Nurse and Palliative Care Teams.

28. Impart sensitive information and advice to patients and carers, which could have life changing consequences, in conjunction with multi-disciplinary team. Demonstrate and apply a broad level of understanding to the effect of hospitalisation and disability.

29. Attend and participate in case conferences and ward meetings appropriate to the clinical setting.

30. Responsible to maintain competence in the use of moving and handling techniques and equipment required for the management of highly dependent patients.

31. Prioritise day-to-day management of own caseload, delegating activities to the Basic Grade OT or the OT support staff, where appropriate.

32. Undertake responsibility of the line management of specific staff. Assist and take responsibility for the organising, support and supervision of Band 5 Occupational Therapists, Occupational Therapy Students and Occupational Therapy support staff within the Team, as required.

33. Responsible for the allocation and organisation of workload within the Team, when the Team Leader is off duty.

34. Adopt a flexible approach in the provision of clinical cover during the absence of other therapists within the team or department.

35. Responsible to take action to ensure defective equipment is withdrawn from service and report the defects to the OT Manager.

36. Contribute to the evaluation and introduction of assessment / treatment media / equipment / tools being used within the team.

Person Specification

Qualifications

Essential

  • Degree or equivalent in Occupational Therapy.
  • To be registered with the Health and Care Professions Council.


Desirable

  • To have qualification/ attendance in relevant clinical areas.
  • Proven evidence of CPD


Aptitude & Abilities

Essential

  • To have demonstrable experience of managing own caseload.
  • To have demonstrable experience of working with other members of the MDT..
  • To have good time management skills.
  • To understand confidentiality.
  • To have good verbal and written communication skills

  • Good manual handling Skills
  • Caseload management skills - especially identifying priorities and meeting deadlines


Knowledge & Experience

Essential

  • To have extensive demonstrable experience in the Physical field in an acute hospital setting
  • To have demonstrable experience of working with Pediatric patients in an Acute Setting.
  • Proven knowledge of the appropriate legislation relating to children and how this applies to occupational therapy
  • To have demonstrable post graduate experience within a large clinical setting.


Desirable

  • To have demonstrable experience in teaching other members of the MDT team both internal and external settings.
