Are you an innovative, creative, motivated caring professional who is looking for a new challenge? We have an exciting opportunity to join our well established Bridport Community Mental Health Team. We welcome applications from newly qualified as a registered professional and require preceptorship.

As a Mental Health Practitioner you will act as a Care Coordinator for a dedicated caseload of patients suffering with Serious Mental Illness with whom you would provide person centred, evidence-based, targeted interventions to aid recovery. You will have a working knowledge of current biopsycho-social models of care and understand how to implement such interventions with your patients to promote optimum health and wellbeing. You will be provided with regular clinical and peer supervision.

The team work collaboratively, you will have the support and opportunity to develop your own skills in evidence-based therapeutic interventions in the psychological therapies as well as advanced nursing.

The Mental Health Practitioner role is a Full Time permanent position, and covers the Bridport, Lyme Regis and surrounding areas. You will need to have a valid driving licence and use of a car as you will be required to review and monitor patients in their own homes and within other community settings. Main duties of the job

You will work closely within our Integrated Multi Disciplinary Team that consists of Consultant Psychiatrists, Mental Health Nurses, an Occupational Therapist, Social Workers, an Advanced Nurse Practitioner, Support Time Recovery Workers, a Physical Health Worker and our Psychology Team. There will be a good deal of work with multiple professionals across a wide range of services.

You will have the opportunity to act as a mentor, assist with induction and in-service training of staff when necessary. As a Mental Health Practitioner maintaining and developing effective links and sound communication with the Inpatient and Crisis Service, particularly in respect of aftercare of discharged patients and ensuring good communications with relatives and other visitors.

Person Specification

Knowledge, skills and training

Essential

  • Registered Mental Nurse / Occupational Therapist/Social Worker
  • Valid registration with NMC or equivalent body
  • Evidence of continuous professional development


Job Specific Experience

Essential

  • Competent with the safe administration of depot preparation injection technique
  • Risk assessment and management
  • Proven communication skills (written and oral)
  • Skills in managing complex cases
  • Works well in a multi-disciplinary team


Desirable

  • Competent computer skills, experience inputting information on RIO
  • Awareness of Primary Care and Steps to Wellbeing.
  • Applies current research and theory into practice
  • Knowledge of clinical Governance and a commitment to clinical supervision.
  • Extensive post graduate experience in working in community mental health
  • Working experience of using the Care Programme Approach
  • Inter-agency working.

  • Demonstrable skills in written and spoken English to a standard which enables the post holder to carry out the full range of duties and responsibilities of the role effectively
