An opportunity has arisen for registered adult nurses to join our innovative, developing & award winning nursing team who has a reputation of achieving clinical excellence within Continuing Healthcare within the community. This is a challenging but rewarding role and you will be fully encouraged and supported in your professional development.
The main duties of the posts is to care for individuals with complex health care needs in their own homes within the community.
The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The roles will be delivering care within packages of care at home and working in a role delivering clinical lead presence across the community ensuring high quality care is delivered. This role will challenge your clinical skills as well as your leadership skills.
We are looking for dedicated individuals who want to deliver truly holistic care to our patients in their own home. You will have the skills to manage individuals with Learning Disabilities and Complex Health needs or the willingness to learn and a willingness to facilitate individuals to participate in all aspects of their life.
We encourage and support continued staff development and have a dynamic internal education nursing agenda which is exciting and aids succession planning. There is also the opportunity to work within the nursing leadership team to develop a wider range of skills.
You will have opportunities to develop a wide range of clinical skills and work with a superb team of nurses and health care support workers.
The ability to drive patient vehicles and the ability to travel between sites in a timely manner is essential
