You will play a key role in supporting the delivery of the Personalisation and Inclusion Pathways for Adults and Carers ensuring legal literacy, strength and asset based practice, personalisation and social inclusion is embedded and applied effectively throughout your work.

You will be responsible for supporting the delivery of health and social care functions for adults/carers within the Community and where relevant Hospital/ Inpatient settings providing social work knowledge and expertise within the domain of assessments care and support planning, reviews and requests for information, advice and guidance

You will have responsibility for your own case load and will work on a range of cases involving assessment, support planning, mental capacity Act assessments, appointeeships and deputyship referrals. Section 117 reviews and safeguarding enquiries as well as complex case management.

You will be expected to hold either an Approved Mental Health Professional, Best Interest Assessor or Practice Educator qualification or the willingness to work towards obtaining one of these.

This is a part-time post of 30 hours per week, which can be agreed to suit the successful candidate within the hours of Monday - Friday 8.30am - 5.30pm. There is opportunity to manage your own workload and diary and you will be offered the opportunity to work in a hybrid way, being based between the office base in Stafford and home working.

Main duties of the job

As a level 2 social worker, you should be highly skilled and experienced and you will be required to be self-motivated and organised in order to manage any competing demands and be provided with all relevant training and support in the role.As part of this role you will be expected to:
  • Assess access and implement a range of social care services, and providing guidance/advice and information for adults with acute or increasingly complex long term physical health, mental health needs and for older people and carers.
  • Work collaboratively with other Trust services to provide seamless care for adults/carers on defined care and support pathways with particular emphasis on ensuring safe and effective transitions.
  • Uphold the principles of wellbeing and ensure people can access their rights and entitlements including access to social care resources under a range of relevant statute and eligibility criteria including the Care Act (2014), The Mental Health Act (1983/2007), the Mental Capacity Act (2005) and the Human Rights Act (1998). To ensure appropriate governance and application of appointeeships and/or advocacy are engaged in accordance with need, rights and entitlements.
  • Proficient at recording patient information and utilising computer systems.

Person Specification

Qualifications & Training

Essential

  • Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body
  • Ensuring current registration with the Professional Social Work regulatory body at all times.
  • To hold/to undertake and achieve the status of Approved Mental Health Professional and/or Best Interest Assessor as per the SWLA Career Pathway
  • To undertake and achieve the status of Work Based Supervisor and Practice Educator as per the SWLA Career Pathway
  • Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint
  • An ongoing commitment to personal and professional development.
  • A clear understanding of and commitment to equal opportunities and anti-discriminatory practice


Desirable

  • Existing accreditation relating to Practice Educator/Work Based Supervisor


Experience

Essential

  • Significant post-qualification experience of working with adults with needs associated to older age, physical health or mental health needs and their carers


Desirable

  • Experience of social work in other user groups/settings i.e. Children and Families, Learning Disabilities, Older Adults Experience of a development role for service/ facilities
