The Harrow Community Therapy Service is looking for an enthusiastic, motivated, experienced candidate who is looking to develop their knowledge and skills within a friendly team. Exceptional support is readily available with a robust supervision & CPD structure. You need to be a team player as well as an autonomous practitioner.

You will receive professional guidance and direction from senior Therapists (Occupational Therapists and Physiotherapists). As part of your development, there are opportunities to experience colleagues working in a range of partner settings, and to develop your supervisory skills with Therapy Support Workers and students.

You will be working closely with the other internal Integrated and Specialist Community Services, including, Community Nursing, Speech and Language, Tissue Viability, Falls and Rapid Response services. You will also be required to work closely with Primary Care Network Colleagues, Frailty Service and the Hospital Discharge Team to support-system flow.

It is also essential to hold a full valid driving licence and have access to a car to use regularly for business purposes.

You will be required to complete electronic patient records (training provided)

The position is full time (37.5 hours per week) but in line with our flexible working approach we welcome applications for part time contracts. A seven-day working pattern will be required to cover weekends and bank holidays on a rostered/rotational basis.

Main duties of the job

  • Undertake specialised uni-professional and multi-professional assessments.
  • To develop and implement treatment plans for clients. These clients will present with an exacerbation of a frailty related condition and will likely have Multiple co-morbidities including neurological conditions, musculoskeletal conditions, osteoporosis, learning disability and/or mental health diagnosis.
  • Participate in the Assessment Process when appropriate, performing overview assessments and complex specialist assessments as appropriate including the ordering of moving and handling equipment relevant to rehabilitation need.
  • To communicate relevant medical information, assessment details, advice and recommendations across agencies and with clients in compliance with Trust Guidelines for sharing of information and informed consent. These agencies include General Practitioner's, Social Services, Healthcare professionals, voluntary agencies, and residential homes.
  • Be responsible for gaining valid, informed consent for assessment, treatment interventions and sharing of information in compliance with legal and professional guidelines. To be able to interpret and follow these guidelines in circumstances where clients do not have capacity to provide informed consent.
  • Have working knowledge of clinical guidelines, specific to the area you are working in and, with the guidance of your supervisor, to translate these guidelines into clinical practice

Essential

  • Diploma/degree in Physiotherapy
  • Current HCPC registration
  • Evidence of up to date CPD portfolio
  • Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation
  • Member of Professional Body


Desirable

  • Member of a Special Interest Group relevant to this post


Experience

Essential

  • Experience post registration as a Band 5 physiotherapist
  • Experience working at a junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory

  • Experience working with voluntary organisations. ? Experience in service development projects.

Essential

  • Skills in the assessment and treatment of adults with complex multipathologies
  • Knowledge and understanding of complex medical conditions including multipathologies and falls

  • Ability to apply core Physiotherapy skills across a range of clinical conditions within a client centered, goal setting
  • Knowledge of outcome measures and evidence-based practice

  • Basic IT skills - Microsoft Word, Power point, Excel
  • Teaching of junior staff / students, peers in Inservice training setting
