We are seeking an enthusiastic nurse to join the acute respiratory nursing service/hospital at home service based at Darlington Memorial Hospital. Our nursing team is made up of a Respiratory Nurse Consultant, Respiratory Matron, a Respiratory Nurse Specialist/Team Lead, a number of Respiratory Nurse Specialists, and Respiratory Health Care Assistant.

Main duties of the job
The role includes a range of duties such as
-providing a 7 day hospital at home service for patients suffering from an exacerbation of COPD. (This is currently in review and can be discussed further)
-Reviewing patients in hospital.
-Contributing to the management of patients admitted with respiratory disease in the form of assessment for home oxygen therapy, making recommendations in line with local and national treatment guidelines, promoting the benefit of pulmonary rehabilitation, smoking cessation advice and disease education.
-The role also includes outpatient clinics for nebulised antibiotics, and biologic therapies for severe asthma, as well supporting the Respiratory Nurse Consultant and Band 8 Lead respiratory Nurse with our nurse led sleep related breathing disorder clinics.

Job responsibilities
MAIN DUTIES AND RESPONSIBILITIES
Assess patients admitted to hospital with respiratory problems, assessing, supporting and facilitating discharge as appropriate including completing the COPD bundle.
Review patients in their own home following acute exacerbations of COPD.
Ensure all patients with COPD and asthma leave hospital with a self-management plan, the most appropriate inhaler device, education and follow up arrangements.
Support patients with chronic obstructive pulmonary disease, asthma and other respiratory conditions on discharge and in the community when appropriate.
Educate patients, carers and staff about the use of equipment and treatments related to respiratory disease, such as inhalers, peak flow meters and nebulisers. To assess knowledge and concordance with medication.
Liaise with community services including community based respiratory nurses, community matrons, and intermediate care services.
Actively participate and provide a Pulmonary Education programme for hospital and community staff to ensure that all professionals receive consistent information.
Attend respiratory medicine meetings.
Develop close working relationships with the wards including Ambulatory care, A& E, medical admissions unit and the base wards.
Assess patients requirements for oxygen on discharge and prescribe as required.
Participate in audit.
Assess patients with a diagnosis of OSA, initiate treatment, educate and advise appropriately.
Follow up patients on CPAP therapy, adjust treatments and troubleshoot as necessary.
