In your new role you will work closely with occupational therapists, Assistant practitioners, support staff, clinical assessors ( UCR team), district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes.

Main duties of the job

An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management, palliative care support and support timely discharges from our acute Trust partners.

You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient's independence, safety and quality of life.

The post will involve cross site working and community visits. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Job description
Job responsibilities

You will manage your own caseload in the community. As part of the integrated care team you will work closely with the rest of the MDT to provide the best possible outcomes for our service users in Mid Nottinghamshire.

You will have a degree/diploma in physiotherapy & be HCPC registered. You should possess excellent interpersonal and time management skills, be a good communicator and able to prioritise and work well as a member of a team. An induction period and supernumerary support will be given. Ideally you will have experience of working in the UK and being part of our healthcare services.

  • BSc/Diploma in Physiotherapy
  • HCPC Registration
  • Clinical Educator

Desirable
  • Relevant courses in Health Care of the Elderly/Falls/Orthopaedics/Palliative Care/MSK

Previous Experience
Essential
  • Proven post qualification experience with some at a more senior level
  • Recent experience in health care of the elderly /falls /orthopaedics /palliative care
  • Supervisory skills
  • Experience of audit.
  • Experience of working in the UK as part of the NHS

  • Teaching experience
