Older peoples Mental Health Liaison nurse, Band 6

We have a vacancy for a band 6 Mental Health Liaison Nurse in the older adult's mental health liaison team. We are based in the Queen Elizabeth hospital and provide support for patients in the emergency department and on the inpatient wards.

This is an excellent opportunity to be part of a dynamic, proactive and caring team. The post is for a full time post working shifts on a rota basis, Monday to Friday 8am-8pm and 9-5 on weekends and Public Holidays.

The successful applicant will have an understanding of the nature and impact of mental health issues particular in relation to older people. You will need to have excellent communication skills and the ability to lead and work proactively and flexibly within the team. You will need to be skilled in mental health assessments and have a clear understanding of assessing and managing risk.

You will need to have experience as a qualified mental health nurse and experience of working with older adults would be an advantage but not essential.

Main duties of the job

To carry out timely and effective biopsychosocial assessments of individuals referred to the team from the Emergency Department, and wards within the acute trust.

To consider all necessary sources of information in formulating the assessment and care plan.

To work as part of a multi-disciplinary team in carrying out effective risk assessment and management, formulating appropriate and safe care plans.

To carry out assessments of mental capacity and use this to inform appropriate treatment decisions.

To liaise and consult effectively with all necessary personnel from the acute trust, Oxleas mental health teams, other mental health providers, community health services and the emergency services.

To communicate effectively and in a respectful manner with service users and carers.

To arrange the safe and timely transfer of clients to Oxleas in-patient wards and other hospitals and units as required.

To be responsible for the full biopsychosocial and risk assessment of clients presenting with a wide variety of mental health conditions. Following this, the post holder will be responsible for the formulation and planning of care / treatment.

To take into account all necessary sources of information in formulating the assessment and care plan.
