Are you what we are looking for? Do you consider yourself to be the next generation ofleaders in the NHS, If so an exciting opportunity has arisen for a Band 6 Nurse in the OuterNorth West Division for a community nurse to work closely within the planned care ofNursing and Therapy.

This is a great opportunity to develop your understanding of quality drivers and to work closely with clinical teams to support the delivery of high-quality care and positive patient and staff experience.We are looking for a dynamic and motivated individual with nursing experience who canprovide local clinical leadership for the multi-disciplinary team working across the Division.

The successful candidate will have the opportunity to further develop quality expertise andlead key projects and pieces of work by developing strong relationships and network acrossthe Division. You will work collaboratively with the wider multi-disciplinary team, otherhealth and social care practitioners and the nursing staff, to provide the best care andquality outcomes for patients and service users, in-keeping with national guidelines.

Main duties of the job

The post holder will work in collaboration with the designated Deputy Team Leader in managing the care of patients with long term conditions, incorporating monitoring progress and promoting the preventative aspects of nursing practice.

Job Purpose
  • To provide skilled nursing care working as part of Central London Community nursing service topatients in a variety of community settings between the hours of 0830 and 2100 hours, includingweekends.
  • To actively monitor, maintain and develop the service provided.
  • To take responsibility for managing the caseload and the team in the absence of the DN team lead.
  • To deliver a case managed approach for patients with long term care needs.
  • To actively participate in service development activities

Education/Qualification

Essential

  • RGN/RN with current registration
  • Evidence of recent and continuing professional education and training
  • Practice supervisor/assessor training


Desirable

  • Has undertaken formal training in staff and team management
  • Nurse prescribing qualification


Experience

Essential

  • Experience of negotiating and liaising within a multi-disciplinary team
  • Experience of working with people with complex health needs
  • Experience of supervising junior staff and co-ordinating a team on a day to day regular basis
  • Experience of teaching students and other staff members
  • Experience of of implementing evidence based care in practice
  • Experience of undertaking comprehensive health needs assessment and writing care plans.


Desirable

  • Experience of managing staff
  • Post-registration community nursing experience


Skills & Knowledge

Essential

  • Principal of care for patients with long term conditions, based on local/national guidelines and standards
  • understanding of NHS and integrated care boards structures
  • The principles of clinical governance and application to practice
  • Able to identify own and others learning and development needs and actively seek ways to meeting those needs.
