We are seeking to recruit an experienced mental health practitioner to be an integral part of the Mental Health Integrated Community Service (MHICS) for Slough. The postholder will be responsible for working closely with General Practitioners and the primary care team to provide biopsychosocial assessments, brief interventions and care navigation for people with significant mental health issues living in the locality.
This is an exciting opportunity to be involved in the development and shaping of a major new and innovative approach to providing mental health care within the community, working closely with all relevant providers within the individual's local community network.
Frimley Integrated Care System has been awarded new NHS funding to transform community mental health services for people with significant mental health problems. A ground-breaking new model is being implemented within Berkshire Healthcare NHS Foundation Trust and Surrey and Borders Partnership NHS Foundation Trust to benefit those patients within the primary care setting.
The funding has enabled new Primary Care Network (PCN) mental health teams to be developed. Each PCN, comprising a population of approximately 30 - 50,000 patients, hosts a multi-disciplinary team which will work in close collaboration with a wide range of stakeholders to provide support for people with significant mental health difficulties to meet their needs, closer to their communities and homes.

Main duties of the job
- Working as a senior level practitioner, supervising junior staff members and coordinating the day-to-day functioning and delivery of the clinical service.
- Working with people with significant mental health problems, whose mental health needs can be best met within primary care and whose difficulties are best understood within a biopsychosocial model.
- Building and facilitating closer links between primary care networks and Community Mental Health Teams, Social Care teams, community services and a wide range of voluntary sector organisations, to achieve seamless transitions and support for people with significant mental health needs.

- Current professional registration: Registered Mental Health Nurse, Social Worker, Occupational Therapist or equivalent
- Evidence of relevant post registration training and development in particular motivational interviewing, brief solution focussed interventions, cognitive and behavioural approaches and psychosocial interventions
