We are looking to recruit a community mental health nurse or Mental health practitioner or Occupational Therapist who has a special interest in Dual Diagnosis for the Mendip Neighbourhood Older persons mental health service, 3 days a weekThe successful applicant will be providing assessment and treatment for individuals who have severe and enduring mental health problems, this could also be alongside coexisting substance misuse disorders. The applicant can be based either in Chantry Day hospital in Frome or Willow Bank, Priory health park, Wells.All training and support will be provided and would suit someone who is looking to develop from a band 5 to a band 6 We are happy to discuss flexible working requirements.

Main duties of the job

The postholder will work as a key worker within the Mendip locality ensuring the provision of a high quality service, working with individuals over the age of 65 years supporting with moderate to severe mental health difficultiesTo act as a keyworker for a caseload of people with mental health difficulties and developing their care plan, coordinating and reviewing their care and treatment outlined in the plan, at times offering direct individual and group interventions to these and other patients where appropriateThe postholder will carry a clinical caseload and will provide supervision and advice to other mental health staff in relation to their working practice with this service user groupThe post holder will promote awareness and best practice, actively supporting positive attitudes and a consistent approach towards assessment and treatment of this service user group using evidenced based interventions and treatment frameworksTo participate in wider multidisciplinary working across primary and secondary care servicesTo promote health through provision of health education, information and advice

To provide assessment, planning, implementation and evaluation of interventions for people presenting with a severe and enduring mental health problem alongside Dual Diagnosis within a designated geographical area or as required by the ServiceTo work within the principles of the Trusts Dual Diagnosis Policy and to help with early identification of people who might benefit from the service and or who might be diagnosable with coexisting disordersTo act as a key worker for a caseload of, assessing their needs, and developing their recovery care plan, Dialog plus, coordinating and reviewing their care and treatment outlined in the Dialog plusTo coordinate working practices amongst other relevant professionals in the designated locality, including psychologists and psychological therapistsTo develop and maintain ones own clinical skills and expertise in the area of approaches to, and treatment of people with a mental health difficulty, Dual Diagnosis and, to be reflective and responsive to meaningful service user and carer involvement.To be part of the teams duty rota

Person Specification

Qualifications

Essential

• RMN OR Diploma/Degree in Occupational Therapy or Social Work or other relevant health or social care professional qualification
• Professional registration with the NMC or HCPC
• UK Driving licence with assess to a car for work purposes
• Willingness to attend short course in Dual diagnosis
• Commitment and evidence of Continuing Professional Development

Desirable

• Knowledge within dual diagnosis across a range of procedures underpinned by relevant psychological /psychosocial theory.
• Evidence of ability to work at core competencies level 3 of the Closing the Gap Dual Diagnosis capabilities framework
• Good understanding of national guidance and the evidence base around dual diagnosis to underpin practice or willingness to learn
• Understanding of drug and alcohol service provision and the relationship required with mental health services

Experience

Essential

• Expertise within older adult mental health, including use of relevant assessments
• Significant experience in working with people with complex clinical and risk presentations, including those diagnosable with dual diagnosis

Desirable

• Experience of offering clinical supervision to other professionals
• Working with clients in the community
• Working within drug and alcohol services or clear partnership with
