We are looking an enthusiastic and highly motivated worker, who shares our vision of making positive difference in the lives of our residents. Hence, we require a community mental health nurse, a social worker or an occupational therapist who is able to work under pressure and enjoy fast-paced work in a dynamic, supportive and friendly environment.

This role involves screening, assessing and providing short-term intervention to local residents who require support from mental health services. The role is structured around the neighbourhood model of care, which is about understanding the needs of local communities, building supportive social networks and strengthening the resilience of service users. The service is made up of practitioners from different professional backgrounds and we work closely with GPs, the voluntary sector and other teams within the mental health service to ensure that residents' needs are met.

You need experience of working with adults with serious and enduring mental health issues using the strength-based model. You will need to have good understanding of the principles of the Care Act, Mental Capacity Act and Mental Health Act and experience of safeguarding vulnerable adults.

Main duties of the job

To provide a high quality and effective mental health support and care to Hackney residents. To respond to the needs of adults with mental disorder and their carers in a timely manner, within the boundaries of statutory legislation, and with regard to individual circumstances. To assess need using a person-centred approach and effectively asses and manage risk. To plan and deliver outcome focused interventions linked to promoting independence & social inclusion. To act as an allocated worker taking responsibility for the provision of community services to mental health services users and carers.

To undertake comprehensive assessment of needs under the Care Act 2014

Knowledge of current national and local legislation that impacts on mental health services.

To prioritise cases as needed, while also meeting assessment targets.

To ensure that service users are assessed and signposted or referred on to appropriate services within a timely manner.

To carry out risk assessments and ensure that safe and appropriate decisions are made in conjunction with MDT.

To establish good working relationships with local GP practices and health centres in order to better meet the mental health needs of individual clients

Essential

• Professional nursing, social work or OT qualification
• Evidence of continual professional development
• Current registration

• Post qualifying experience of delivering relapse prevention work and specific psycho-social interventions

Experience

Essential

• Experience of working in a secondary mental health community team as a qualified member of staff implementing the recovery model of care.
• Experience of making full psycho-social assessments and writing reports.

• Experience of providing support and education to carers/families and supporters

Skills and ability

Essential

• Ability to engage with people with serious mental health problems in a holistic manner, promoting recovery and client-focused care plans.
