The post holder will demonstrate detailed specialist knowledge of both type 1 and type 2 diabetes. They will act as a source of specialist advice in the nursing management of patients in their specialist area who require a high level of judgment, discretion and decision-making whilst as an inpatient/outpatient or in a setting external to the Trust. The post holder will provide education and training to patients, carers and healthcare professionals.

Main duties of the job

Assess, plan, priorities, implement and evaluate nursing interventions to meet patients' needs.

Be competent in all areas on Continuous Glucose Monitoring Systems , to include fitting the sensors group teaching to both staff and patients. . Be competent with downloading and interpreting the date and also with using different diabetes platforms available .

Undertaken comprehensive healthcare needs of patients, changing treatment plan as necessary.

Be responsible for assessing and recognizing emergencies in their specialty. Able to interpret information and take appropriate action and lead others to do so. Provide specialist care, knowledge and expertise, whilst being continually aware of the importance of empowering colleagues to ensure dissemination of knowledge and best practice.

Attend consultant led outpatient clinics and report on cases (as appropriate to the role).

Run nurse/midwife led clinics (as appropriate to the role.

Provide highly specialist advice on the phone, liaising with general

practitioners, district nurses and hospital medical teams as appropriate.

To take a lead role in the organization and management of multi

professional patient care.Support the specialist service to patients attending the hospital.

Contribute to and support the work of colleagues in the multi-disciplinary team.

Person Specification

Experience

Essential

  • Minimum of 2 years practising as a diabetes nurse
  • Must have NMP qualification
  • Be competent with all Continuous glucose systems
  • have experience with both primary and secondary care in relation to diabetes management
  • Must be willing to work across sites including Wethington, Wythenshawe and Trafford
  • Be included in audit in relation to diabetes and diabetes care/safety


Desirable

  • ability to use both Hive and Emis recording systems
  • Be Dafne or Desmond trained to deliver patient structured nice accredited education
  • have delivered formal education to other health care employees in relation to diabetes and its management


minimum 2 yrs band 6 diabetes nurse
