An exciting opportunity has arisen for a dynamic individual to provide excellent patient care within our Cardiovascular Prevention and Rehabilitation Service. We are looking to appoint a part-time Band 6 Cardiac Rehabilitation Specialist Nurse to the Royal Devon University Healthcare Trust - North Devon service.
The successful candidate will join our multidisciplinary team, working in collaboration to motivate and inspire our patients and their carers to engage in improving their health and recovery following a cardiac event.
The aim is to provide menu-based services closer to the patients home.
Based on the British Association of Cardiovascular Prevention and Rehabilitation (BACPR) Standards and NICE guidance, we provide high quality care focused on improving patient's health and quality of life through health education, advice on cardiovascular risk reduction, physical activity and stress management.
The successful candidate should have a considerable wealth of cardiology knowledge, clinical experience and leadership skills. Excellent communication skills and a commitment to motivate an effective team are essential.

As a Band 6 Cardiovascular Prevention and Rehabilitation Clinical Nurse Specialist you will provide a high standard of evidence-based care to patients and their families/carers following their cardiac event.
You will be required to use analytical and investigative skills to undertake a comprehensive, specialist assessment of the patient, referring patients to other members of the MDT where appropriate.
Assess, plan, deliver and evaluate patient centred care. This will include symptom management, risk factor management, medication optimisation and education to support patients lead healthier lives and promote self-management skills.
You will contribute to monitoring and improving standards of care through supervision of practice, clinical audit, developing and leading practice, contributing to research and teaching.
You will be required to support patients within the District General Hospital and community hospitals settings, and at our community venues. Therefore ability to drive and your own car is essential.
Informal visits are welcomed and encouraged.
Working Pattern: 22.5 hours per week provisionally set to 7.5hr days Tuesday, Thursday and Friday this pattern may be changed due to needs of the service.
