The post holder will be part of a dynamic team of clinicians whose responsibility it is to develop and improve services for people over 18 years with common mental health problems in primary care. The role includes working across designated GP practices within the primary care networks, attending weekly integrated care team meetings.
The post holder will provide effective consultation and liaison across the primary and secondary care sectors, whilst facilitating and improving access to appropriate services. The role is to provide mental health screening assessments, risk assessment, medication monitoring, telephone triage, sign posting, referral to other services and brief intervention for people experiencing common mental health problems,
The post holder will also provide support, education and problem-solving advice to other members of the local primary care teams particularly in the areas of assessment and management of mental health problems, working across different geographical settings and undertake home visits.

Main duties of the job
The post holder will undertake specific managerial, administrative, preceptor and clinical supervisor roles as required by the post. To ensure up to date, accurate clinical notes and activity recording relevant to the role and participate in audit and any appropriate approved research.
In response to current local policy and national strategy for mental health, active involvement in developing services to meet the mental health needs of users and carers in primary care, ensuring they are equitable, non-stigmatising and responsive to local need and available to people aged 18 and over experiencing a common mental health problem. To maintain and improve knowledge and skills based on best practice particularly specialist knowledge and skills in mental health issues in primary care. To act as role model and resource for junior/less experienced staff and students from the multi-disciplinary team.

Job responsibilities
- Provide triage and assessments of mental health needs using sound knowledge base and evidence-based outcome measures in all areas of mental health including risk assessment and risk management
- To work in partnership with service users to identify effective strategies to promote understanding and management of presenting mental health problems
- Provide information, advice, education, health promotion and liaison for referrers, service users and carers, colleagues, and the wider community about all aspects of mental health and the options, choices, and opportunities available to them
- Critically analyse service user information, formulate individual plans of care, using evidence based brief interventions.
- To signpost and support referral of service users to a range of services, as appropriate including primary, secondary and voluntary agencies and to follow up where required
- To participate in the development and delivery of primary care based therapeutic and support groups in response to local need
- To attend and activity participate in the multidisciplinary integrated care team meetings weekly in surgeries across the borough
- To actively participate in appropriate managerial, clinical and caseload supervision for self in accordance with Trust policy in order to support all clinical work undertaken
- Where identified through assessment, facilitate the stepped care model/clinical pathways for clients experiencing enduring mental health problems

- oEvidence of current continued professional development
- oWillingness to undertake non-medical prescribing
- oPost registration experience of working autonomously in community mental health services
- oExperience of working collaboratively within a multi-disciplinary/ multiagency team
- oExperience of managing and organising own workload
- oSkills in assessing and formulating individual intervention plans and outcome measures for adults with mental health problems
