This is an exciting opportunity for experienced Mental Health Nurses to support the Adult Mental Health LMHTs as part of the community transformation programme, in line with the longer-term plan for community services. The community transformation includes opportunities to develop skills within SMI pathways (mood affective, complex psychosis, community rehabilitation and non-psychosis including the Cluster 8 and Trauma Pathway).
We are particularly interested in hearing from people with skills and experience, working with adults with a range of mental health difficulties who require complex case management/clinical interventions.
You will receive a full local induction into the role.
Main duties of the job
You will be expected to work with patients and their families to support triage of referrals, assessment, personalised care planning and in the provision of specialist interventions. You will promote the recovery of patients and through working with partner agencies and primary care to ensure timely and safe discharge from secondary care.
This is a leadership role, and you will be expected to develop specialist skills in working within all pathways within the team. You will also be expected to support students and more junior staff within the team through clinical supervision and mentorship, and you may be asked to line manage some staff.
We are seeking enthusiastic team players, to work within a dynamic multi-disciplinary health team, with excellent communication and interpersonal skills, and the ability and experience to manage complex cases in partnership with other agencies.
