Barnet Planned service provide community Nursing services to Barnet CCG and is split into three locality teams. This post holder will be covering one of the permanent post in the South of the Barnet locality. The post holder will work closely with the locality manager, team leads/ other clinicians to develop the service and maintain close working relationships with both acute and community services.

Main duties of the job

You need to be compassionate person, able to show empathy for patients and their carers. You will need to be punctual, pro-active, have a flexible approach to work and be able to deliver high quality of nursing care in the community setting.

Essential: It is essential to have a current clean driving licence (less than 6 points) and have access to your own car.

You need to be able to use a map and SAT NAV to navigate to patient's homes. Please confirm in the supporting information that you have a car and clean driving licence

Qualifications

Essential

  • Registered Nurse with current UK NMC registration
  • Evidence of recent and continuing post-registration professional education and training equivalent to post-graduate diploma level


Desirable

  • Mentor training
  • Has undertaken formal training in staff and team management
  • Nurse prescribing qualification


Experience

Essential

  • Evidence of recent post-registration nursing experience
  • Experience of negotiating and liaising within a multi-disciplinary team
  • Experience of working with people with complex health needs
  • Experience of supervising junior staff and co-ordinating a team on a day to day, regular basis
  • Experience of teaching and mentoring others including students
  • Experience of implementing evidence-based care in practice
  • Experience of undertaking comprehensive health needs assessment and writing care plans


Desirable

  • Community nursing and/or primary care post-registration nursing experience
