We have an exciting opportunity to recruit a band 5 Occupational Therapist to our community stroke service in the west of the county
Our service provides Early Supported Discharge (ESD) for both Stroke and Acquired Brain Injury (ABI) patients across Somerset and specialist inpatient and ongoing community stroke rehabilitation.
Our service has received excellent feedback from patients, carers and commissioners, as well as high levels of staff satisfaction and has been awarded an innovation award by the Community Hospitals Innovation Team. We strive to use current best practice and do so by regularly reviewing our service and adapting to provide patients with the best possible outcomes using evidence-based practice.
Main duties of the job
The Community Stroke Rehabilitation Team offers Early Supported Discharge and Stroke Specific Rehabilitation to patients in both Stroke Rehabilitation Units and in patients own homes or normal place of residence.
Our Mission is to deliver Specialist, Goal Orientated Rehabilitation which empowers people, and those important to them, to live well after Stroke.
