With the national emphasis being on supporting patients rehabilitation in their own homes and communities, this is an exciting time to join Somerset Foundation Trust Community Rehabilitation Service in a senior clinical role.

This is an excellent opportunity for a senior, experienced therapist who wishes to develop their own clinical skills and those of less experienced staff. This post encompasses the Enhanced Clinical Practitioner Framework to allow the post holder to gain ECP accreditation, providing a clear clinical career pathway. The post holder will link directly and receive support from experienced clinicians who are already in place in community services.

As a Physio Clinical Specialist in Community Rehabilitation, you will be part of an MDT consisting of Physios, OTs, Pharmacy Technicians and Support Staff, working alongside the District Nurses and Older Persons Mental Health Teams as we continue with Integration of community services. You will link closely with the Advanced Clinical Practitioners and liaise with other agencies such as, Primary Care, Adult Social Care and voluntary agencies.

Main duties of the job

We provide planned rehabilitation for residents over the age of 18 in Taunton and West Somerset.
  • The caseload is varied from the straightforward mobility assessment to highly complex frailty, post-acute neuro-rehabilitation, orthopaedics and falls.
  • There is specialist support with neuro conditions and excellent links with complex MDT clinics.
  • People are visited in a host of settings, including their own homes, local community, community hospitals and care homes. You will be expected to support the most complex cases through your own and shared caseloads.

You will have advanced therapy assessment and clinical reasoning skills and be able to liaise with primary, secondary and tertiary stakeholders.

Qualifications

Essential

  • BSc Hons or equivalent Physiotherapy degree qualification
  • Relevant Post graduate study
  • Teaching qualification or equivalent experience Mentoring
  • Willing to undertake Enhanced Level training through an apprenticeship of individual module basis


Experience

Essential

  • Proven leadership and management experience with community therapy patients
  • Proven evidence of educational support to others


Desirable

  • Experience in a clinical leadership role
