An exciting opportunity has arisen for 2 Qualified Band 6 Mental health practitioners to join The Fylde Frequent Attenders Team. This post will cover Blackpool, Wyre and Fylde localities.

Service users within the Frequent Attender Programme are high intensity/complex individuals with multiple issues and needs. They can be frequent attenders within the Emergency Department or come to the attention of emergency services such as the police and be subject to multiple Sc 136s. They may frequently utilise other emergency services such as the police or NWAS. Individuals may will also be receiving care from Home Based Treatment Team or CMHT.

We need staff who are highly motivated and proactive and who are able to work assertively and empathically with people within the locality to encourage their engagement with multi agency services and support them within their recovery. This will involve working collaboratively with other agencies to aid engagement to meet the needs of the service user.

The B6 MHP's will provide a comprehensive assessment and develop collaborative care plans with Service Users, their families and other agencies and where necessary signpost to other services within the community. For those Service Users that are already known to mental health services you will work collaboratively with the Key Worker to develop a robust plan of care (or dovetail care plans) that Multi agencies are involved with to support the person and prevent crisis.

Main duties of the job

Working hours: Monday to Friday 0900-1700.

• You will provide a comprehensive assessment and develop collaborative care plans with Service Users and other agencies and, where necessary signpost to other services within the community. Referrals will come via the Frequent Attenders database for the locality. For those Service Users that are already known to Mental health services you will work collaboratively with the Care Coordinator to develop a robust plan of care that Multi agencies are involved with to support the person and prevent crisis. If they are unknown you will case manager for a short period of time to enable referrals to the relevant organisation/agency.
• This comprehensive, collaborative triage system will lead to comprehensive assessment clear onward facilitation of service users to the most appropriate care following assessment.
• The post holder will provide evidence based mental health assessments and/or signposting to alternative service provision for people who present requiring a specialist urgent mental health assessment. The post holder may offer follow-up appointments within the agreed timescale for intervention as part of the on-going assessment process.

Essential

• Assertiveness skills in relation to bed management.
• Demonstrate the ability to work sensitively in a non judgemental way with service users who are in crisis .

• Complete risk assessments which reflect the risk a service user may pose. Transfer the risk assessment into robust risk management planning.

Knowledge

Essential

• Able to understand the difference between crisis planning and contingency planning and able to formulate explicit planning in these areas .
• Knowledge, understanding and experience of the care pathways for psychosis and non-psychosis and relevant interventions .
• Knowledge of Safeguarding procedures, Criminal Justice procedures, Mental Health legislation, guidance and processes .
• Understanding of how physical illness and substance misuse can affect the mental health of individuals .

Qualifications

Essential

• Registered Mental Health Practitioner.
• Social Work equivalent qualification.

Desirable

• Practice teacher qualification/student supervisor.

Work Related Circumstances

Essential

• Ability to meet the travel requirements for the needs of the role.
• Ability to work a 24 hour shift pattern.
