Jasmines Ward (Tolworth Hospital) is a 16 bedded unit (male & female) that offers mental health assessment, care and treatment for patients with dementia (including young onset) and/or severe and enduring mental health problems.

At Jasmine Ward, we provide outstanding holistic person-centred care for patients who cannot be safely supported in the community. Jasmines ward adopts a comprehensive and integrated approach to care, with a multidisciplinary team jointly managing all aspects of physical health, mental health, social care and risk/ safety management. Our goal is to provide a well-planned, collaborative, and holistic system of care. Jasmines ward embraces a Triangle of Care approach - involving patient's loved ones throughout their inpatient journey.

Jasmine's ward is a forward-thinking ward embracing digital solutions to support care provision (e.g. electronic medication management electronic therapeutic observation and engagement) and is keen to progress quality improvement initiatives.

Main duties of the job

You need to have a genuine passion in working with elderly and frail patients and willingness to make a difference in their lives. You should be an individual who adopts a proactive approach to care. This role is pivotal in supporting the team in delivering care.

As a pivotal member of our team, you will contribute to the delivery of high standards of care, incorporating a recovery-oriented approach that encompasses all aspects of health needs and risk management. This includes working closely with relevant stakeholders (e.g. community mental health in aging services, social care, advocacy services, third sector organisations) to ensure a safe environment and providing support to individuals in meeting their psychological and physical health requirements.

At Jasmines ward we are keen to find candidates who are eager to develop their nursing skills. Alongside trust-wide training opportunities (E.g. preceptorship, supervision and annual appraisal), we can offer you bespoke local training (e.g. cognitive testing catheterisation, phlebotomy, ECG, weekly simulation lead by the ward psychologist, wound management training, bespoke bitesize training and dementia training.

We provide career development opportunities that support staff to remain within the pathway (working with older adult), to develop as a clinician and from inpatient to community team. This could be as a specialist nurse in Memory Assessment Clinic or working within any of our challenging behaviour services.

Job description

Job responsibilities

  • Leading on and undertaking activities to ensure that the fundamentals of care are met - nutrition, hydration, hygiene, comfort, emotional and social support.
  • Co-ordinating shifts and ensure the smooth running of the ward.
  • Co-ordinating and evaluating collaborative patient-centered care getting feedback and improving care plans with the multi-disciplinary team (MDT).
  • Providing accurate information about care in an accessible format to patients and their relatives / carers.
  • Liaising and working positively with members of the multi-disciplinary team and partner organisations, to ensure that appropriate and individual treatment is provided.
  • Administering, and monitoring the side-effects of, medication monitoring service usercompliance.
  • You will also have opportunities to work with student nurses (in a supervisor role and possibly as an assessor if qualified).

    Education and Qualification

    Essential

    • Registered Adult Nurse and/or Registered
    • Mental Health Nurse and/or Registered
    • Learning Disabilities Nurse

    • Mentorship qualification


    Experience and Knowledge

    Essential

    • Experience of research-based/reflective practice, e.g. understanding and ability to use supervision to reflect on skills, attitude and knowledge and develop these by using evidence based practice.
    • Knowledge of relevant Professional and Clinical legislation


    Desirable

    • Experience of shift co-ordination.
    • Experience of supervising and mentoring junior staff and students


    Skills and Abilities

    Essential

    • Able to provide quality care that is responsive to service user's needs, without close supervision
    • Resilient: able to cope with difficult interpersonal situations.
    • Advocates for all individuals, particularly those with protected characteristics promotes equality and diversity.

    • Skilled in supervision of others, and in providing critical and constructive feedback.
