An exciting opportunity has arisen for an experienced dietitian to join our dynamic and friendly Community Dietetics Team in Oldham. The senior managers in the care organisation in Oldham are inclusive, friendly and encourage innovation.
You will work as an autonomous practitioner within in a variety of settings including community clinics, care and nursing homes and patients own homes. You will work as the key professional to ensure seamless discharge of patients from acute to community settings.
You will support patients who are discharged before seen by the acute dietitians and A& E to prevent further deterioration in their condition, readmission via A& E and reliance on community services, you will liaise with Community Dietetics team, local GP's, care agencies and Urgent Care Team in the proactive management of complex patients with a view to prevent admission to hospital.
You will work with multidisciplinary teams to reduce attendance at A& E, improve independence and advice on appropriate use of oral nutritional supplements thereby reducing unnecessary spending on supplements you should have sufficient experience and advanced clinical decision making skills to enable you to work independently across both Acute and Community Dietetics teams and Urgent Care.

Main duties of the job
You will work with patients across a range of specialities including enteral tube feeding, gastro -intestinal conditions, complex respiratory conditions, motor neurone disease, adverse symptom management and severe malnutrition.
You will be based primarily in community. However, you will have strong liaisons with the acute and community settings to ensure timely, safe discharges of appropriate patients. Your role will include high level of communication between acute and community dietetics and other relevant MDT members.
We are involved in the training of student dietitians and staff development is a high priority. We are committed to work-based education and training to improve clinical skills and we actively encourage CPD and audits.

- BSc/MSc Degree in Nutrition & Dietetics
- Registered as a Dietitian with the HCPC
- Relevant accredited BDA courses

- Membership of the BDA specialist group

- Relevant clinical experience of enteral feeding and oral nutrition support
